<br />State-paid positions under this contract Attachment shall perform activities as specified in <br />PERFORMING AGENCY'S FY 02 Service Delivery Plan. Activities shall be specific to one or <br />more of the ten essential publi~ health services or cardiovascular and cancer activities. <br /> <br />FINANCIAL ASSISTANCE <br /> <br />PERFORMING AGENCY shall direct General Revenue or 73.5% of the funds toward the <br />delivery of a broad range of essential public health services and the Preventive Health and Health <br />Services Block Grant or 26.5% of the funds for activities designed to reduce the incidence of <br />cancer and cardiovascular disease. <br /> <br />PERFORMANCE MEASURES <br /> <br />The following performance measure(s) will be used to assess, in part, PERFORMING <br />AGENCY'S effectiveness in providing the services described in this contract Attachment, <br />without waiving the enforceability of any of the other terms of the contract. <br /> <br />For the General Revenue funds the PERFORMING AGENCY shall participate in one of the <br />three stages of the National Public Health Performance Standard (NPS) Model specified in the <br />attached PERFORMING AGENCY'S Service Delivery Plan. The three stages include: <br /> <br />1. NPS Survey Stage: PERFORMING AGENCY shall complete the NPS survey and submit <br />the completed survey to the appropriate RECEIVING AGENCY Regional Director no <br />later than August 31, 2002. <br /> <br />2. Systems Improvement Plan (SIP) Design Stage: PERFORMING AGENCY shall develop <br />a Local Public Health SIP and submit the developed SIP to the appropriate RECEIVING <br />AGENCY Regional Director no later than August 31, 2002, An enhanced model of a <br />system improvement process may be performed, such as Assessment Protocols for <br />Excellence in Public Health (APEXP H), Mobilizing for Action through' Planning and <br />Partnerships (MAPP also formerly known as APEXP H) or other system improvement <br />process. The system improvement process shall be approved by the appropriate <br />RECEIVING AGENCY Regional Director. <br /> <br />3. SIP Implementation Stage: PERFORMING AGENCY shall implement and evaluate the <br />SIP, and submit findings and recommendations to the appropriate RECEIVING <br />AGENCY Regional Director no later than August 31, 2002. <br /> <br />For the Preventive Health and Health Services Block funds the PERFORMING AGENCY shall <br />complete activities in the approved Service Delivery Plan to reduce cardiovascular disease and <br />cancer in the PERFORMING AGENCY'S jurisdiction no later than August 31,2002. <br /> <br />SECTION II.
