Paris—Lamar County Health District <br />400 West Sherman Street, Paris, Texas 75460-5646 <br />Health District: (903) 785-4561 <br />Women, Infant and Children (WIC): (903) 784-1411 <br />VAR15ILMAR COUNTY Fax: (903) 737-0978 <br />UEAk"I IDI <br />0 ANYAK-1, r151amar-1 <br />Minutes of Regular Meeting <br />Paris - Lamar County Board of Health <br />The Paris - Lamar County Board of Health held a regular meeting at 5:30 p.m. on Monday, January 2204, 2018, <br />Board members in attendance were Mr. Bill Collins, Dr. Mark Gibbons, Dr. Eric Salzman, Mrs, Nancy Stallings, Dr. Marc <br />White, and Dr, Ross Williams. Others in attendance were Mrs, Gina Prestridge and Ms, Cheryl King. Not in attendance was <br />Mrs. Connie Staulter, <br />Mr. Collins called the meeting to order in open session at 5:37 p.m, <br />Dr. Eric Salzman made the motion to approve minutes from prior meeting and was seconded by Dr. Marc White. All approved, <br />Mrs. Prestridge asked the board to revise and adopt the Food Establishment Grading System to include a $20.00 late fee if <br />not paid within 30 days of receipt. Also, if original fee and late fee is not received with 90 days then the facility will be subject <br />to closure and all' charges for re -inspection will apply. All approved, <br />Mrs. Prestridge advised that in the past we have paid McClanahan and Holmes to prepare the W-2 and 1099's for the health <br />department at a cost of $500,00 but would like to pay the payroll officer at the health department to prepare them this year and <br />pay her $2,50.00. All approved, <br />Mrs. Prestridge updated the board on, the DSRIP funding that is available and the expected due dates. <br />The board members choose to wait until next board meeting and go into executive session to discuss and act on Mrs. Gina <br />Prestridge and Dr. Amanda Green yearly evaluations, <br />October, November, and December reports — <br />* WIC Services 13451' 1299 / 1279 clients served (140 / 118 / 107) drive thru <br />* 10641' 521 1425 Clinical clients served <br />* 41 / 32118 restaurant inspected with 4 / 1 / 0 complaints <br />* 811 4 septic systems inspected with 1 / 2 / 0 complaints and <br />* 010 0 rabies cases. <br />0 Total liability and equity is approximately $927,000.00. <br />The next regular meeting would be February 26th, 2018, at 5:30 p.m. at the Paris - Lamar County Health District, <br />Business to be discussed and possibly action taken on at next meeting: Yearly Evaluations <br />Dr. Gibbons made the motion and was seconded by Dr. Williams the meeting to be adjourned at 6:14 p.m. r R <br />All approved, 1A <br />fml <br />Respectfully submitted by: <br />Cheryl D.
