SERVICES <br /> <br />TERM: September 01, 1997 <br /> <br />THRU: August 31, 1998 <br /> <br />SECTION I. SCOPE OF WORK: <br /> <br />ESSENTIAL PUBLIC HEALTH SERVICES <br /> <br />PERFORMING AGENCY will use direct assistance and/or fmancial assistance, as specified in <br />SECTION m., BUDGET, from RECEIVING AGENCY to deliver one or more of the following <br />ten (10) essential public health services as specified in PERFORMING AGENCY'S FY'98 <br />Service Delivery Plan, which is herein adopted by reference: <br /> <br />1. Monitor health status to identify community health problems; <br />2. Diagnose and investigate health problems and health hazards in the community; <br />3. Inform, educate, and empower people about health status; <br />4. Mobilize community partnerships to identify and solve health problems; <br />5, Develop policies and plans that support individual and community health efforts; <br />6. Enforce laws and regulations that protect health and ensure safety; <br />7. Link people to needed personal health services and assure the provision of health <br />care when otherwise unavailable; <br />8. Assure a competent public health and personal health care workforce; <br />9. Evaluate effectiveness, accessibility, and quality of personal and population-based <br />health services; and <br />10. Research for new insights and innovative solutions to health problems. <br /> <br />Two types of support are provided under this program: (1) direct assistance in the form of State- <br />paid positions and/or vaccines and/or (2) financial assistance from General Revenue funds and the <br />Preventive Health and Health Services Block Grant. <br /> <br />STATE-PAID POSITIONS <br /> <br />These positions report to and are directly supervised by PERFORMING AGENCY administrative <br />staff. Supervision authorization includes, but is not limited to, overseeing daily work assignments <br /> <br />ATTACHMENT - Page 1 <br />
