PERFORMING AGENCY will provide copies of investigation <br />forms to the RECEIVING AGENCY. <br /> <br />PERFORMING AGENCY will adhere to the Vaccine-Preventable Disease Surveillance Guidelines <br />provided by the RECEIVING AGENCY. PERFORMING AGENCY will implement the most <br />current outbreak control procedures and measures as recommended and provided by RECEIVING <br />AGENCY. <br /> <br />PERFORMING AGENCY will implement an immunization reminder and recall system to notifY <br />parents or guardians of children when immunizations are due. The notifications may be automated <br />or manual and may include mailed or telephone contacts. Extra efforts shall be made to notify <br />parents or guardians of children at high-risk of failure to complete the vaccines on schedule (e.g. <br />children who start their vaccines late). <br /> <br />PERFORMING AGENCY agrees to provide immunization services outside usual clinic hours <br />(8:00 a.m. to 5:00 p.m.) at least once each month or as needed to insure barrier-free access to <br />immunization clinics. PERFORMING AGENCY residency requirements are not applicable under <br />this Attachment. PERFORMING AGENCY cannot deny vaccinations to recipients because they <br />do not reside within PERFORMING AGENCY's jurisdiction. <br /> <br />PERFORMING AGENCY agrees to maintain an accurate, up-to-date list of clinics and sites <br />where public sector (free or low cost) immunization services are offered in PERFORMING <br />AGENCY'S local area. PERFORMING AGENCY further agrees to update the clinic list monthly <br />and provide the updates to local area Aid to Families with Dependant Children (AFDC) offices <br />and to the Immunization Communication and Training Program of RECEIVING AGENCY. <br /> <br />PERFORMING AGENCY will provide immunization services in public clinics regardless of <br />ability to pay. <br /> <br />No fee may be charged for vaccines provided by RECEIVING AGENCY. All vaccines obtained <br />under this Attachment must be used solely for purposes of this Attachment and cannot be sold to <br />agencies or individuals. <br /> <br />PERFORMING AGENCY shall not collect vaccine administration fees from Medicaid recipients. <br />Vaccine administration fees collected from non-Medicaid patients will be kept within guidelines <br />established by RECEIVING AGENCY and described in "Vaccines for Children", available at <br />immunization clinics. No one may be denied immunization services because of inability to pay the <br />administration fee.
