<br />PERFORMING AGENCY agrees to implement the "Standards for Pediatric Immunization <br />Practices," as recommended by the National Vaccine Advisory Committee, approved by the <br />United States Public Health Service, and endorsed by the A.merican Academy of Pediatrics. <br />PERFORMING AGENCY agrees to provide immunization services outside usual clinic hours <br />(Sam to 5pm) or on week-ends at least once each month or as needed to insure barrier free access <br />to immunization clinics. PERFORMING AGENCY residency requirements are not applicable <br />under this Attachment. PERFORMING AGENCY cannot deny vaccinations to recipients because <br />they do not reside within PERFORMING AGENCY's jurisdiction. <br /> <br />PERFORMING AGENCY will provide an estimated 17.500 clients with services/units of service <br />in or benefiting the county(ies)/area defined as: Lamar. <br /> <br />SECTION II. SPECIAL PROVISIONS: <br /> <br />General Provisions, REPORTS AND INSPECTIONS Article, is amended to include the following <br />paragraph: <br /> <br />PERFORMING AGENCY agrees to submit an Annual Expenditure Report to the appropriate <br />Public Health Regional Director no later than sixty (60) days following the end of PERFORMING <br />AGENCY'S fiscal year in a format prescribed by RECEMNG AGENCY. <br /> <br />PERFORMING AGENCY agrees to maintain an accurate, up-to-date list of clinics and sites <br />where public sector (free or low cost) immunization services are offered in PERFORMING <br />AGENCY'S local area. PERFORMING AGENCY further agrees to update the clinic list monthly <br />and provide the updates to local area AFDC offices and to RECEIVING AGENCY Program's <br />Communication and Training Section to assist with client referral. <br /> <br />PERFORMING AGENCY agrees to assist in distributing state-supplied vaccines to "Texas Health <br />Steps" providers, Medicaid providers, physicians, and other providers and organizations within <br />PERFORMING AGENCY'S local area. <br /> <br />PERFORMING AGENCY agrees that fees will be collected in accordance with 25 TAC ~1.91. <br />and that no one may be denied immunization services because of inability to pay the fee. Fee <br />schedules will not be based on vaccine type, formulation, or dose in series. <br /> <br />PERFORMING AGENCY understands that travel funds are provided for use by persons assigned <br />to the immunization project. All out -of-state travel and travel for persons not assigned to the <br />immunization project (this Attachment) require prior approval by the RECEMNG AGENCY, <br />Immunization Division Director. <br /> <br />ATTACHMENT - Page 5 <br />
