00) Number and type of employees, and hours of operation of plant and proposed or actual <br />hours of operation of pretreatment system; <br />(11) Type and amount of raw materials processed (averaged and maximum per day), which <br />are known to contain or exhibit a potential to produce any limited, restricted or prohibited <br />substance or toxic pollutants; <br />(12) Any other information as may be deemed by the city to be necessary to evaluate the permit <br />application. <br />The city will evaluate the date farriished by the users and may require additional <br />information. After evaluation and acceptance of the date furnished, the city may issue a <br />wastewater contribution permit subject to the terms and conditions provided herein. <br />(c) Permit conditions. Wastewater contribution permits shall be expressly subject to all provisions <br />of this article and all fees established by the city. Permits for all significant industrial users as <br />defined in this article shall contain the following minimum conditions and requirements; other <br />users may be subject to any or all of the following requirements: <br />(1) Statement ofd ation-, <br />(2) Statement of non -transferability; <br />(3) Effluent limits based on applicable pretreatment standards, categorical pretreatment <br />standards, local limits, and state and local law; <br />(4) Specifications for monitoring programs including and not limited to: sampling locations, <br />frequency of sampling, number, types of pollutants to be monitored, and standards for <br />testing and reporting schedules; <br />(5) Requirements for notification of SLUG discharges; <br />(6) Requirements for federal categorical pretreatment standards for users subject to such <br />standards; <br />(7) Requirements for submission of technical reports or discharge reports; <br />(8) Requirements for maintaining and retaining plant records relating to wastewater <br />discharge as specified by the city, and affording the city access thereto; <br />
