All submittals must follow the same format No exceptions to this format will be accepted. To be accepted for <br />evaluation, the submittal format must address all required components in order. <br />The aim of the required format is to simplify the submittal preparation and evaluation process and to ensure that all <br />submittals receive the same orderly review. <br />All submittals must include the following components: <br />A. Cover Letter and Firm Profile <br />B. Signature Affidavit <br />C. Attachment I — Conflict of Interest Questionnaire <br />D. Attachment II — Bid Form <br />E. Attachment M — Resolution 2020-052 <br />a. Appendix A — Safety Record Application <br />b. Appendix B —Safety Record Survey, Projects #1-5 <br />Submittal Comonents <br />A. Cover Letter and Firm Profile: Provide the following information about your firm: <br />• The firm's name, a -mail address, business address, phone number and fax number <br />• Types of services and products offered <br />• Number of years in business <br />• Number of employees <br />• Names of supervisors to be assigned to this project <br />• The location of the offices that would provide the project services <br />• A brief statement of the firm's background, demonstrating longevity and financial stability <br />• Current workload <br />• Demonstrate the ability of your firm to complete deadlines. <br />B. Signature Affidavit <br />C. Attachment I — Conflict of Interest Questionnaire <br />D. Attachment H — Bid Form <br />E. Attachment III — Resolution 2020-052 <br />Selection Criteria <br />If a bid is submitted in which there is a material failure to. comply with the Submittal Format and Selection Criteria, <br />such bid shall be rejected and the contract shall be awarded to the next lowest responsible bid conforming to said <br />Submittal Format and Selection Criteria. <br />The City of Paris may use the following selection criteria to evaluate bids prior to awarding a contract: <br />1. The price; <br />2. The offeror's experience and reputation; <br />3. The quality of the offeror's goods or services; <br />4. The impact on the ability of the governmental entity to comply with rules relating to historically underutilized <br />businesses; <br />5. The offeror's safety record; <br />6. The offeror's proposed personnel; <br />7. Whether the offeror's financial capability is appropriate to the size and scope of the project; and/or <br />8. Any other relevant factor specifically listed in the request for bids, proposals, or qualifications. <br />Following a review of submittals, it is the intent of the City that a contractor will be selected.
