l 1. Site: Contractor is responsible for keeping the project area clean during construction and is responsible for <br />final cleanup of project site. <br />12. Locations: The City of Paris Public Works Department will assign project areas as needed in various <br />locations. <br />13. Down Time: The City of Paris will direct the work in coordination with the contractor to expedite completion <br />of work and to minimize any down time and moving time. <br />14. lnstectipn: The City of Paris Public Works Department will inspect the project site on a daily basis. <br />15.quantities: Quantities shown in this RFB are estimated quantities only. <br />16. Point ,of Contact: The contractor selected for this assignment will work closely with City staff. The Street <br />Superintendent, or his designee, will serve as the primary point of contact and will be responsible for <br />coordinating and implementing the subsequent work. Other officials who will be involved in this project <br />include, but are not limited to, the Public Works Director and the City Engineer. <br />Submittal Format <br />All submittals must follow the same format. No exceptions to this format will be accepted. To be accepted for <br />evaluation, the submittal format must address all required components in order. <br />The aim of the required format is to simplify the submittal preparation and evaluation process and to ensure that all <br />submittals receive the same orderly review. <br />All submittals must include the following components: <br />A. Cover Letter and Firm Profile <br />B. Signature Affidavit <br />C. Attachment I — Conflict of Interest Questionnaire <br />D. Attachment II — Bid Form <br />E. Attachment III — Resolution 2020-052 <br />a. Appendix A — Safety Record Application <br />b. Appendix B —Safety Record Survey, Projects #1-5 <br />Submittal Comte onents <br />A. Cover Letter and Firm Profile: Provide the following information about your firm: <br />• The firm's name, e-mail address, business address, phone number and fax number <br />• Types of services and products offered <br />• Number of years in business <br />• Number of employees <br />• Names of supervisors to be assigned to this project <br />• The location of the offices that would provide the project services <br />• A brief statement of the firm's background, demonstrating longevity and financial stability <br />• Current workload <br />• Demonstrate the ability of your firm to complete deadlines. <br />B.
