SANCHEZ, M.D. , M.P.H. 1100 W. 49U' Street • Austin, Texas 78756 <br />COMNIISSIONER 1- 888 - 963 -7111 • http:flvnw.ddshs.st f �Veu <br />NOV 18 2005 <br />TO: Local Registrar a4CoClerks City off P�arriis <br />ty FROM: Geraldine R Ha State Registrar <br />Texas Vital Stati <br />DATE: November 9, <br />SUBJECT: Fee Increases <br />The Texas Department of State Health Services has officially amended Texas Administrative Code <br />Title 25 Chapter 181 changing the fee Texas Vital Statistics charges for the following search and <br />issuance of vital records: <br />Birth search and certificate $22.00 each <br />Heirloom birth certificate $60.00 each <br />Death search and first certificate $20.00 <br />Additional death certificates ordered at the same time $3.00 each <br />Marriage and Divorce search and verification $20.00 <br />Commemorative wedding certificate (new product) $60.00 <br />The new fees become effective December 1, 2005. As required by Texas Health and Safety Code <br />Chapter 191.0045 local registrars must charge the same fees as charged by the state office. <br />Local registrars and county clerks may collect a fee not to exceed $1.00 for the preservation of vital <br />statistics records under Texas Health and Safety Code 191.004(h). This fee is collected by the registrar <br />on the issuance of a vital statistics record. This dedicated preservation fee may be in addition to the <br />$22.00, $20.00 or $3.00 fee per certificate. <br />An Equal Employment Opportunity Employer <br />
