SERVICES <br /> <br />TERM: September 01, 1998 <br /> <br />THRU: August 31, 1999 <br /> <br />SECTION I. SCOPE OF WORK: <br /> <br />ESSENTIAL PUBLIC HEALTH SERVICES <br /> <br />PERFORMING AGENCY shall use direct assistance and/or [mancial assistance, as specified in <br />SECTION m., BUDGET, from RECEIVING AGENCY to deliver one or more of the following <br />essential public health services as specified in PERFORMING AGENCY'S FY 99 Service <br />Delivery Plan, which is adopted by reference: <br /> <br />. Monitor health status to identify community health problems; <br />. Diagnose and investigate health problems and health hazards in the community; <br />. Inform, educate, and empower clients about health status; <br />. Mobilize community partnerships to identify and solve health problems; <br />. Develop policies and plans that support individual and community health efforts; . <br />. Enforce laws, rules, regulations, and ordinances, where applicable, that protect <br />health and ensure safety; <br />. Link clients to needed personal health services and assure the provision of health <br />care whe,n otherwise unavailable; <br />. Assure a competent public health and personal health care work force; <br />. Evaluate effectiveness, accessibility, and quality of personal and population-based <br />health services; and . <br />. Research for new insights and innovative solutions to health problems. <br /> <br />Two types of support are provided under this program: (1) direct assistance in the form of <br />state-paid positions and/or (2) [mancial assistance from General Revenue funds and the Preventive <br />Health and Health Services Block Grant. <br /> <br />, STATE-PAID POSITIONS <br /> <br />State-paid positions under this contract Attachment shall perform activities as specified in <br />PERFORMING AGENCY'S FY 99 Service Delivery Plan. Activities shall be specific to one of <br />the ten essential public health services or cardiovascular and cancer activities. <br /> <br />ATTACHMENT - Page 1 <br />
