<br />How to Get Reimbursed <br />Claiming your before-tax dollars to pay covered expenses is an easy process. Remember you must have at least $25 <br />in expenses to be reimbursed. In addition, the medical care must be provided during the plan year for which you <br />have set up your account. <br /> <br />Your expenses will be reimbursed up to the amount you have pledged for the year in your Umeimbursed Health <br />Care Spending Account. The total yearly amount is available for reimbursement as soon as the plan year starts and <br />the expense incurred. <br /> <br />Step 1: <br />When you have a covered medical expense, obtain a receipt showing that date of service and the service provided <br />(you do not have to pay for the service before submitting it for reimbursement). <br /> <br />Before applying for reimbursement, submit any medical bills covered by insurance as you normally would to any <br />insurance company that covers you or your dependents. IRS allowable expenses not reimbursable by insurance can <br />then be submitted for reimbursement. <br /> <br />If the service provided is for a TML IEBP covered employee or dependen~ the amount reimbursable by flex will <br />automatically process a flex check after the claim is processed. If the service is covered under another insurance <br />policy, submit a copy of the Explanation of Benefits from that insurance company along with a Flex Reimbursement <br />Form for reimbursement (A copy of the form 'is included in this booklet). <br /> <br />If you are emolled in both an Unreimbursed Health Care Spending Account and a Health Savings Account, your <br />Umeimbursed Health Care Spending Account will not reimburse you for any allowable expenses applied toward <br />satisfaction of your medical plan deductible. If you are enrolled in a Health Savings Account, expenses applied <br />toward your medical plan deductible can be reimbursed only under you Health Savings Account. Except, if your <br />medical plan deductible is more than the minimum deductible established by federal law for a qualified high- <br />deductible health plan, after you have satisfied the minimum deductible required under federal law, either your. <br />Unreimbursed Health Care Spending Account or your Health Savings Account may be used to reimburse expenses <br />applied to your deductible that exceed the federally-established minimum. <br /> <br />Step 2: <br />Mail your completed reimbursement claim form and documentation to: <br />TML Intergovernrnental Employee Benefits Pool <br />PO Box 140167 <br />Austin, Texas 78714-0167 <br /> <br />Step 3: <br />You will receive a spending account reimbursement check made out to you and mailed to your home address. <br />Claims are paid within 10 working days from the date of receipt. <br /> <br />PRIVACY OF YOUR HEALTH INFORMATION <br /> <br />A Federal regulation, called the Privacy Rule, requires the Plan Sponsor of a Umeimbursed Health Care Spending <br />Account to protect the privacy of your and your dependent's health information. The Plan Sponsor and the Plan <br />Supervisor take their responsibilities to protect your health information seriously and will use and disclose <br />individually identifiable health information only when needed to pay claims submitted for reimbursement under the <br />Health Care Spending Account, when needed to administer the Health Care Spending Account or when required by , <br />law. The Privacy Rule prohibits the Plan Sponsor from using or disclosing any health information from the Health <br />Care Spending Account for employment-related actions and decisions, or for the administration of any other <br />employee benefit plan of the Plan Sponsor. <br /> <br />In addition to restrictions on how the Plan Sponsor and Plan Supervisor may use and disclose individually <br />identifiable health information, the Privacy Rule gives you and your covered dependents certain rights. These rights <br />include the right to access your health information, to amend (or correct) your health information and to receive an <br />accounting of certain disclosures of your health information. <br /> <br />Page 9 <br />
