<br />The final report, FSR, form 269A (Form GC-4a), shall be submitted no later than ninety <br />(90) days following the end of the applicable term. <br /> <br />General Provisions, General Business Operations of Contactor Article, Section 12.03, <br />Management and Control Systems, Item a, regarding internal budgeting and Item c, regarding <br />billing, collection, and fee schedules are not applicable to this contract Attachment. <br /> <br />General Provisions, General Business Operations of Contractor Article, Section 12.07, <br />Overtime Compensation, is not applicable to this contract Attachment. <br /> <br />General Provisions, General Business Operations of Contractor Article, Section 12.21, <br />Equipment Purchases, is replaced with the following: <br /> <br />Contractor shall accept responsibility and financial liability for all equipment and <br />supplies purchased with DSHS funds, whether purchased locally by Contractor or by <br />DSHS and transferred to the care and custody of Contractor. Contractor shall conduct an <br />annual physical inventory of all equipment purchased by DSHS and submit it at a time to <br />be specified by DSHS. DSHS reserves the right to recover the cost of equipment or <br />supplies purchased by DSHS and placed in the custody of Contractor if such articles are <br />lost, stolen, or otherwise unaccounted for. <br /> <br />General Provisions, General Business Operations of Contractor Article, Section 12.27, <br />Property Acquisitions, is not applicable to this contract attachment. <br /> <br />SECTION IV. BUDGET: <br /> <br />All categories of costs billed to DSHS WIC Program, and allocation of such costs, shall be in <br />accordance with the "Plan to Allocate Direct Costs" (P ADC) submitted by Contractor and <br />approved by DSHS WIC Program. This document is incorporated herein by reference and made <br />a part of this contract Attachment. <br /> <br />ATTACHMENT - Page 9 <br />
