<br />"(2) Statement of non-transferability; <br /> <br />"(3) Eflluent limits based on applicable pretreatment standards, <br />categorical pretreatment standards, local limits, and state and local <br />law; <br /> <br />"(4) Specifications for monitoring programs including and not limited to: <br />sampling locations, frequency of sampling, number, types of <br />pollutants to be monitored, and standards for testing and reporting <br />schedules; <br /> <br />"(5) Requirements for notification of SLUG discharges; <br /> <br />"(6) Requirements for federal categorical pretreatment standards for users <br />subject to such standards; <br /> <br />"(7) Requirements for submission of technical reports or discharge <br />reports; <br /> <br />"(8) Requirements for maintaining and retaining plant records relating to <br />wastewater discharge as specified by the City, and affording the city <br />access thereto; <br /> <br />"(9) Requirements for notification of the City before any new introduction <br />of wastewater constituents or any substantial change in the volume <br />or character of the wastewater constituents being introduced into the <br />wastewater treatment system; <br /> <br />"(10) Statement of applicable civil and criminal penalties for violations and <br />any applicable Compliance schedules. Such schedules may not <br />extend the compliance date beyond applicable federal deadlines; <br /> <br />"(11) Any other conditions may be included as deemed appropriate by the <br />City to ensure compliance with this article. <br /> <br />"(d) Signatory requirements, All reports or other required documentation shall <br />be signed and certified by the authorized representative of the permit holder. The <br />certification statements shall be worded as follows: <br /> <br />"I certify, under penalty of law, that this document and all <br />attachments were prepared under my direction or supervision in <br />accordance with a system designed to assure that qualified personnel <br />properly gathered and evaluated the information submitted. Based on <br />my inquiry of the person or persons who manage the system, or <br />those persons directly responsible for gathering the information, the <br />information submitted is, to the best of my knowledge and belief, <br />true, accurate, and complete.
