<br />DOCUMENT NO. 7560022067 -01 <br />AITACHMENTNO.03A <br /> <br />PERFORMING AGENCY: PARIS-LAMAR COUNTY HEALTH DEPARTMENT <br /> <br />RECEIVING AGENCY PROGRAM: BUREAU OF NUTRITION SERVICES <br /> <br />TERM: October 01,2000 THRU: September 30, 2001 <br /> <br />It is mutually agreed by and between the contracting parties to amend the conditions of Document <br />No. 7560022067 2001-03 as written below. All other conditions not hereby amended are to remain <br />in full force and effect. <br /> <br />SECTION 1. SCOPE OF WORK, is revised to include the following: <br /> <br />M. Increase the competency and knowledge level of WIC staff in order to improve the quality <br />of services to eligible women, infants and children. Strengthen the provision of nutrition <br />services, including nutrition assessment, screening and referral, and nutrition education, by <br />enhancing the knowledge and skills of staff. <br /> <br />N. Implement mandated reporting requirements promulgated by the General Appropriations Act <br />for fiscal years 2000-2001 to include Rider 18 relating to the screening and reporting of child <br />abuse in compliance with Chapter 261, Texas Family Code. <br /> <br />SECTION II. SPECIAL PROVISIONS, paragraphs one and two under section titled <br />PARTICIPANTS SERVED PER MONTH MAXIMUM REIMBURSEMENT, are replaced with <br />the following: <br /> <br />PARTICIPANTS SERVED PER MONTH MAXIMUM REIMBURSEMENT: <br /> <br />During the term of the Attachment, PERFORMING AGENCY will earn administrative funds at the <br />rate of $.8.,[Z for each participant served as defined above. <br /> <br />· PERFORMING AGENCY shall provide immunizations under the WIC Special Initiative through <br />September 30,2001, unless otherwise notified in writing by RECEIVING AGENCY. Immunizations <br />administered and funded under the WIC Special Initiative to WIC participants/applicants and their <br />siblings will earn administrative funds at the rate of $5.07 per immunization as reported to <br />RECEIVING AGENCY via the Texas WIN automated system. The per-shot rate is based on actual <br />cost. When the study of cost-per-shot shows a different actual cost, then the rate of compensation <br />paid per-shot will change. The PERFORMING AGENCY will be notified of the change and will <br />have thirty (30) days to provide written notice to the RECEIVING AGENCY if it intends to <br />terminate this contract. <br /> <br />A IT ACHMENT - Page 1 <br />
