<br />MEDICAL TRANSPORT AGREEMENT <br /> <br />STATE OF TEXAS <br /> <br />) <br />) <br />) <br /> <br />KNOW ALL MEN BY THESE PRESENTS: <br /> <br />COUNTY OF LAMAR <br /> <br />THAT, the CITY OF PARIS, PARIS, TEXAS, a municipal corporation, acting by and <br /> <br />through its Mayor, Michael J. Pfiester, duly authorized, hereinafter called CITY, and DUBUIS <br /> <br />HOSPITAL OF PARIS, acting by and through its Administrator, Berry Gilbert, duly authorized, <br /> <br />hereinafter called HOSPITAL, do hereby contract and agree as follows: <br /> <br />1. CITY agrees to furnish transportation of patients to or from the Dubuis Hospital of Paris at <br /> <br />the HOSPITAL'S request. <br /> <br />2. CITY will bill HOSPITAL at the customary rate for transport and supplies, with HOSPITAL <br /> <br />being entitled to a twenty percent (20%) discount if payment to CITY is made within fifteen <br /> <br />(15) days of the billing date. <br /> <br />3. In the event HOSPITAL has not paid bill within thirty (30) days of billing date, one hundred <br /> <br />percent (100%) of bill will accrue interest at the statutory limit. <br /> <br />4. It is hereby mutually agreed that either party has the right to unilaterally terminate this <br /> <br />agreement after having given thirty (30) days written notice to the other party. <br /> <br />EXECUTED this 12th day of August, 2002. <br /> <br />CITY OF PARIS, PARIS, TEXAS <br /> <br />By: <br /> <br />Michael J.
