Brady Environmental Services, Inc. <br />Prior to beginning work, the contractor shall provide the owner with a completed <br />Certificate of Insurance providing the below listed coverage. The owner shall be <br />included as an Additional Insured by Endorsement to policies issued for coverage listed. <br />a.) Commercial General Liability insurance at minimum combined single limits <br />of $2,000,000 per occurrence and $4,000,000 general aggregate for bodily <br />injury and property damage. Coverage must be written on an occurrence <br />form. Contractual liability must be maintained with respect to the <br />contractor's obligations contained in the contract. The general aggregate <br />limit must be at least two (2) times the per-occurrence limit. Shall include <br />coverage for: Premise operations, independent contractors, <br />products/completed operations, personal injury, contractual liability and <br />medical payments. Shall include an endarsement for pollution related tc <br />asbestos abatement and disposal. <br />b.) Workers' Compensation Insurance - The contractor is responsible for <br />both Federal and State Unemployment Insurance coverage and Standard <br />Workers Compensation Insurance coverage in the amount of $1,000,000. <br />Contractor shall comply with all federal and state tax laws and withholding <br />requirements. <br />c.) Texas Business Automobile Policy - Automobile Liability Insurance, <br />including Contractual Liability, covering all motor vehicles owned, hired or <br />used in the performance of this contract, with limits not less than: <br />$1,000,000 Bodily Injury & Property Damage Combined each occurrence <br />and aggregate. <br />The required limits may be satisfied by any combination of primary, excess <br />or umbrella liability insurances, provided the primary policy complies with <br />the above requirements and the excess umbrella is following form. <br />With reference to the foregoing insurance requirement, the successful bidder <br />shall specifically endorse applicable insurance policies as follows: <br />a. The owner shall be named as an additional insured with respect to <br />general liability. <br />b. A waiver of subrogation in favor of the owner shall be contained in the <br />workers compensation coverage. <br />c. All insurance policies shall be endorsed to require the insurer to <br />promptly notify the owner of any material change in the insurance <br />coverage. <br />d. All insurance policies shall be endorsed to the effect that the owner will <br />receive at least thirty (30) days notice prior to cancellation, non-renewal <br />or termination of the insurance and ten (10) days notice for non-payment <br />of premium. <br />9of17 <br />
