<br /> <br /> <br /> Scope of Work - Asbestos Abatement Irerracon <br /> Grand Theatre m 21 Lamar Avenue, Paris, Texas <br /> April 14, 2011 m T erracon Project No. 94107645 <br /> CONSULTANT'S RESPONSIBILITIES: <br /> <br /> <br /> The responsibility of performing the final visual inspection and the final air <br /> clearance is delegated to T erracon's on-site State of Texas licensed project <br /> manager. <br /> <br /> Review submittals within two working days or indicate in writing reasons for reviews <br /> which require additional time. <br /> <br /> Review for conformance with design concept of project and information given in Contract <br /> Documents. <br /> <br /> Indicate results of review and return submittals to CONTRACTOR for distribution. <br /> <br /> CONSULTANT is not responsible for verification of field measurements, construction <br /> criteria, catalog numbers and other similar data. <br /> <br /> <br /> Review of separate items does not constitute review of an assembly in which items <br /> function. <br /> <br /> 5.0 CONSTRUCTION NOTES <br /> <br /> The CONTRACTOR shall be responsible for the submission of the <br /> Demolition/Renovation Notification Form. The CONTRACTOR shall provide proper <br /> written notification (and revisions as required) to the appropriate regulatory agencies on <br /> the agency's designated form. The Notification form shall be complete and accurate and <br /> contain a statement indicating the Scope of Work and accurate quantities of ACM to be <br /> removed. The CONTRACTOR is responsible for the accuracy and timelines of the <br /> <br /> notification. The execution of a Contract/Work Order for this project by the OWNER <br /> shall constitute written authorization for the CONTRACTOR to complete and sign the <br /> Notification on the OWNER'S behalf. CONTRACTOR'S bid will include the cost of the <br /> TDSHS fee and will be paid by the CONTRACTOR. The CONTRACTOR shall be <br /> responsible for payment of notification fees included in the Demolition/Renovation <br /> Notification Form. On Notification form, OWNER shall be listed as stated in the contract <br /> with the OWNER. <br /> <br /> The CONTRACTOR will be responsible for routing water and electricity to the work <br /> areas. Water and electrical service are present on the site at this time. <br /> <br /> The CONTRACTOR is to be current and in good standing on all asbestos abate en <br /> notification fees.
