Scope of Work - Lead Paint Abatement lrerracon <br />Grand Theatre o 21 Lamar Avenue, Paris, Texas <br />April 11, 2011 ru Terracon Project No. 94107645 <br />b) The specific location and number of samples will be determined upon <br />conclusion of the remediation. If at any time the post-abatement or <br />removal wipe samples exceed the clearance levels identified. The <br />Contractor shall be responsible additional cleaning of the area to attempt <br />to achieve clearance levels. If additional cleaning is required, the <br />Contractor will perform the re-cleaning with no adjustment in the contract <br />price. <br />4.0 CONTRACTOR SUBMITTALS <br />Submittals required for proper execution include but are not limited to the following: <br />Pre-Construction Submittals (submitted to CONSULTANT): <br />Regulatory Notification Information <br />Plan of Action <br />Fire Action Plan <br />Emergency Phone List <br />Project Schedule <br />Copy of Written Respirator Program which conforms to 29 CFR 1910.134(b) <br />OSHA Material Safety and Data Sheets (Product Handling) <br />Negative Initial Exposure Assessment <br />Copy of EPA "uniform hazardous waste manifest" form <br />Sample of disposal bag and labels to be used. <br />Construction Submittals (submitted to CONSULTANT before start of work on-site) <br />Licenses/Certifications: Contractor, Supervisor, Transporter(s) <br />List of Certified Lead Supervisor and Workers <br />Medical Examination Results <br />Pre-project lead blood levei monitoring testing for each supervisor and worker <br />Worker Training Certificates <br />Respiratory Fit Test Certificate <br />Proiect Closeout (submitted to CONSULTANT) <br />Contractor's Daily Log <br />Worker Registration Certificates for Personnel <br />Company Licenses/Certifications <br />Worker Training Certificates <br />Respiratory Fit Test Certificate <br />Post-project lead blood level monitoring testing for each supervisor and worker <br />Personal Air Monitoring Lab Results <br />Waste Disposal Manifest Copies <br />in 0 5 <br />
