<br />DRAFT <br /> <br />Department Head requests that the employee complete form FMLA -1 and provides to the employee <br />a form FMLA-2 for the physician to complete. The supervisor and or the Department Head <br />completes form FMLA-3 and gives to the employee to notify the employee of a family and medical <br />leave condition. Notification must occur immediately but no later that 48 hours after the supervisor <br />has received notice of a family and medical leave condition and before the employee returns to work. <br />The beginning leave date will be the first day of leave that the employee took related to the <br />condition. The supervisor may not designate family medical leave after the employee returns <br /> <br />to work from the family medical condition unless it is a chronic medical condition that has <br />previously been designated as a family medical condition or the process of securing information to <br />determine if the illness is a family medical leave condition was initiated before the employee <br />returned to work. <br /> <br />SEcrID..N 14.07 - Military Leave <br /> <br />Any employee who is called to active duty as a member of the National Guard or a military reserve <br />unit of the United States is entitled to leave of absence rights and reinstatement rights in accordance <br />with applicable state and federal laws. <br /> <br />SECTION 14.08 - Administrative Leave with Pa <br /> <br />An employee ordered not to report or who cannot report to work because of inclement weather or <br />disaster may be granted administrative leave with pay by the City Manager. <br /> <br />Employees will be granted sufficient administrative leave with pay when necessary, in order to vote <br />in an official election, or to take job related license examinations. Employees called for jury and/or <br />witness service will be granted administrative leave with pay during such service and will retain any <br />fees paid by the courts. Employees excused or released fromjury and/or witness service will report <br />to their workstation in accordance with departmental instructions if half or more of the day remains. <br /> <br />SECTION 14.09 - Funeral Leave <br /> <br />Upon the death ofa member of an employee's immediate family, the employee may receive up to <br />24 hours per calendar year (three 8 hour work days) off the job to attend the funeral, make any <br />necessary arrangements, travel to and from the funeral if out of town and/or assist in settling the <br />estate. <br /> <br />For purposes of this policy immediate family members include: spouse, child, step-child, parent (or <br />someone who served in a parent!child relationship with the employee), step-parent, sister, brother, <br />step-sister, step-brother, grandparent, step-grandparent, parents-in-law, grandchild, step-grandchild, <br />brother-in-law, sister-in-law, and relatives residing in the employee's home. <br />35 <br /> <br />Rev. 10/27/04 <br />
