<br />DRAFT <br /> <br />Attachment 3 <br /> <br />CITY OF PARIS <br />Health Insurance Portability and Accountability Act <br />Notice of Privacy Practices <br /> <br />THIS NOTICE DESCRIBES HOW MEDICAL INFOMRA TION ABOUT YOU MAY BE <br />USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. <br />PLEASE REVIEW IT CARE FULL Y. <br /> <br />CITY OF PARIS GROUP HEAL Tn'S RESPONSIBILITIES <br /> <br />Under the Health Insurance Portability and Accountability Act of 1996 (HIPP A), the City of Paris <br />(City), as administrator of your group health plan, is required to maintain the privacy of your <br />protected health information (PHI) and to provide you with notice of its legal duties and privacy <br />practices with regard to your protected health information. Protected health information is <br />individually identifiable health information that is transmitted or maintained in writing, electronically, <br />orally, or by any other means that is created or received by a health care provider, health plan, <br />employer or health care clearinghouse and relates to the past, present or future physical or mental <br />health condition of an individual. PHI includes but is not limited to information related to a person's <br />participation in the health plan, the person's physical or mental health, or the payment of health care <br />services received by the person. <br /> <br />CHANGES TO NOTICE <br /> <br />The City is required by law to abide by the terms of the privacy notice currently in effect. The City <br />reserves the right to change its privacy practices and to apply the changes to any protected health <br />information the City received or maintained prior to the effective date of the change. The City will <br />notify all city employees and other participants in the City's health plan of any revised notice of <br />privacy practices. The City will distribute the notice electronically or via regular mail to covered <br />employees and other participants in the City's health plan before the effective date of any changes. <br /> <br />USES AND DISCLOSURES OF PROTECTED HEALTH INFORMATION <br /> <br />The City may use or disclose protected health information, when permitted or required by law. As <br />your group health plan administrator, the City must use and disclose protected health information in <br />order to pay benefits to you and your health care providers. The City uses physical, electronic, and <br />procedural safeguards to protect your personal information from being used or disclosed <br /> <br />69 <br /> <br />Rev. 10/27/04 <br />
