<br />DRAFT <br />PROCEDURAL REMINDERS REGARDING <br /> <br />CONTROLLED SUBSTANCE / ALCOHOL ABUSE SCREENING <br /> <br />When an employee is involved in an on-the-job accident involving a vehicle, mobile equipment <br />(tractors, diggers, etc.), or an injury that is not minor, the employee is required to submit to <br />and pass a controlled substance and alcohol screening process. When the injury is minor in <br />nature it is up to the Supervisor to decide if the testing is warranted. Any time an accident <br />report is completed and the employee involved is not tested, the Supervisor will be required to <br />include why a test was not required on the report. If more than one employee is involved, both <br />should be tested. Examples of this would include: <br /> <br />1. <br /> <br />An accident involving an Equipment Operator and another employee injured by the <br />equipment. <br /> <br />2. <br /> <br />A motor vehicle accident with both driver and passenger(s). (In the case where a fire <br />engine or ambulance is involved only those employees in the cab of the vehicle would be <br />tested.) <br /> <br />If testing is required the supervisor should drive the employee to the testing facility as soon as <br />possible after the accident. It is MANDATORY that the employee not be allowed to operate <br />City equipment/vehicles until a negative result has been received. The supervisor can not <br />physically restrain the employee from driving a personal vehicle however the supervisor can <br />ûrone..ly suuest <br />that it is not advisable until test results have been received. <br /> <br />The signed release should be turned into the Personnel Department as soon as testing is <br />completed. If the accident takes place after hours, the release should be turned in to the <br />Personnel Department no later than 12:00 noon on the next normal business day. <br /> <br />St. Joseph's Hospital should be utilized for testing in all cases except when the injury is serious <br />enough that a hospital stay is in order or St. Joseph's Hospital is not available. <br /> <br />If the accident occurs during normal working hours, the testing facility will Administer <br />appropriate tests, complete testing procedures, and contact the Personnel Department with the <br />results. A Personnel Department representative will then contact the department/division. <br /> <br />If the accident occurs outside of normal business hours, the employees supervisor should <br />complete the verbal test results section on the authorization letter to the testing facility. (See <br />Checklist, Step 5) <br />CARE SHOULD BE UTILIZED WHEN AUTHORIZING THE RELEASE OF THIS <br />INFORMATION AND IT SHOULD BE RELEASED ONL Y TO SUPERVISORY <br />PERSONNEL WITH A NEED TO KNOW. <br />THIS IS CONFIDENTIAL INFORMATION.
