<br /> <br /> <br /> <br /> <br /> <br /> (3) The Persorinel Director is responsible for <br /> turnishing professional assistance to departments for <br /> Section J(1) activities, receiving all test resuits <br /> from the designated laboratory and communicating the <br /> test results to the employee's Department Head. <br /> (4) The Employee Assistance Program is <br /> responsible for providi.rig counseling and referral <br /> services as outlined in the City of Paris Personnel <br /> Policies and Procedures Manual. <br /> (5) T'he City of Paris designated medical tacility <br /> is responsible for obtaining a signed consent form from <br /> the applicant or employee, for medical examination and <br /> coilection of specimens necessary for drug and alcohol <br /> testing in a designated laboratory, for arranging <br /> transportation of the specimeri to the laboratory and <br /> for receiving test results in accordance with legally <br /> and medically approved procedures, methods and <br /> techniques. The medical faciiity will communicate the <br /> test results to the Director of Personnel upon receipt <br /> froni the laboratory. <br /> (6) The City of Paris designated medical facility <br /> is resporisible for maintaining records of all <br /> examinations, tests and results in employee and <br /> applicant medical files and for ensuring privacy and <br /> confidentiality. <br /> K. DISCLOSURE OF TEST RESULTS <br /> All persons associated with the City's drug <br /> testing policy are prohibited from willfully disclosing <br /> test results to City employees who cio not have a need <br /> to know or persons not eligible to receive test results <br /> under applicable law. Persons who violate these <br /> prohibitians will be subject to disciplinary action <br /> which could include dismissal." <br /> Section 2. All ordiriances or parts of ordinances in <br /> contlict herewith are hereby expressly repealed. <br /> <br /> Section 3.
