Fungal-Biological Hazard Remediation Protocol-Phase II <br />Grand Theater ■ 21 Lamar Street, Paris, Texas <br />February 24, 2011 ■ Terracon Project No. 94117645 <br />Irerracon <br />■ Specifications for Accident Prevention Signs and Tags: Title 29, Part 1910, <br />Section 145 of the Code of Federal Regulations. <br />14.2 Texas Department of State Health Services (TDSHS): Texas Mold Assessment and <br />Remediation Rules, 2004 (TMARR). <br />15.0 PROJECT SUBMITTALS <br />Prior to start of work, as a minimum, one copy of the following is to be provided to Terracon by <br />the MRC: <br />■ TDSHS 5-Day (or emergency) notification. <br />■ Contractor's TDSHS license (corporate and supervisor). <br />■ Worker documentation, including evidence that all workers have received proper <br />training and are accredited and registered as required by regulations; respiratory fit <br />test for each worker who is to be at the jobsite. <br />■ Material Safety Data Sheets (MSDS) to be utilized on this project. <br />■ Mold Remediation Work Plan <br />At the conclusion of the remedial project the MRC will submit the following: <br />■ A Certificate of Mold Remediation Damage in accordance with TDSHS regulations <br />contained in the TMARR (25 TAC §295.301-295.338). <br />16.0 MITIGATION OF MOISTURE <br />The building owner is responsible for the mitigation of moisture intrusion and the required <br />remedial actions should be provided to the MAC. <br />➢ Roof repairs must be verified to ensure that moisture intrusion due to roof leaks has been <br />mitigated. <br />Responsive ■ Resocarceful ■ Reliable <br />7 <br />MAC # 0107/11-10-12 <br />
