NCM Demolition and Remediation, LP <br />OSHA VIOLATtON STATEMENT - 3 YEARS <br />The following is the citation history for NCM Demoiition and Remediation, LP (including former names and/or <br />predecessor identities, as applicable). NCM and/or its predecessors have not received any wiilful or repeat <br />citations. <br />NCM Demolition was issued one citation by Maryland Department of Labor, OSHA (December, 2011) (OSHA <br />Activity#315827253). The citation was issued because a#oreman did not use appropriate personal protective <br />equipment when testing a 13.8 kV switchgear cabinet. A reduced penalty of $4200 was paid. OSHA classified <br />the citation as Other. <br />MARCOR Environmental, LP was issued three citations by the California Department of Labor, OSHA (April <br />2011) [OSHA Activity #3156077881. 7wo of the violations related to the hearing conservation program and <br />one violatton related to an inventory of chemicals being used per the hazard communication standard. A <br />reduced penalty of $4,039.20 was paid. OSHA classified all three of the violations as Other. <br />CST Environmental, LP was issued three citations by the Nevada OSHA (February 2011) [OSHA Activity <br />#314888967J. Two citations were related to the use of a floor bufFer to remove mastic without having a proper <br />notification submitted prior to pertorming the activity. The errors were corrected by amending the notification <br />to include the use of a floor buffer. The third citation was issued for failure to perform proper final air <br />ciearance. This was corrected by providing all employees an eight hour asbestos refresher course with an <br />emphasis on final air clearance requirements. A reduced penalty of $1,664 was paid. OSHA classified these <br />violations as Other. <br />CST Environmental, LP was issued one citation by the Ohio Department of Labor, OSHA (December 2010) <br />[OSHA Activity #395039453] concerning adequate use of fall protection equipment. Superoisor overseeing <br />selective interior demolition failed to properly mark opening in floor, a potential fall hazard. Materials were <br />immediately obtained and the opening was properly secured during time of inspection. Retraining was held for <br />supervisor and employees on properly securing potential fall hazards. A reduced penalty of $3,500 was paid. <br />CST Environmental, LP was issued one citation by the Ohio Department of Labor, OSHA (December 2010' <br />[OSHA Activity#394100637] concerning dust control on a demolition project. The number of fire hoses used <br />during the demolition process was increased to suppress the creation of dust. Proper PPE's were used at all <br />times to protect employees from exposure to dust. No fine was issued. OSHA classified this violation as Other. <br />CST Environmental, Inc, was issued one citation by the Industrial Commission of Arizona, Division of <br />Occupational Safety and Health (March 2010) [OSHA Activity #314384363J related to an incomplete <br />decontamination unit installed at a regulated work area. The cited conditions were immediately corrected to <br />the inspector's satisfaction. The citation was issued as non-serious without monetary penalty. OSHA <br />classified this violation as Other. <br />CST Environmental, LP was issued three citations by the Florida Department of Labor (March 2010) [OSHA <br />Activity#314495225]concerning housekeeping. One citation was related to items not kept cleared from work <br />areas, passageways exposing employees to possible trip, fall or cut hazards. The demolition debris was <br />immediately removed from the work area and properly disposed. Two citations related to the temporary use of <br />an extension cord (or hoses) in an inactive stairway to perform the work. The cords were immediately <br />removed from the stairuvay when identified as a potential trip hazard. Workmen were advised to secure cords <br />(or hoses) overhead or outside the potential path of travel. A reduced penalty of $3,037 was paid. <br />
