Obtaining Gas Service <br /> <br /> A. Application for Gas Service <br /> Company has the right to require written application for Gas Service. <br /> <br /> (1) Reasons for Refusal of Application for Gas Service <br /> Company may decline service to an Applicant for whom service is available from <br /> previously installed facilities until such Applicant has complied with the state and <br /> municipal regulations and approved rules and regulations of Company on file with the <br /> applicable Regulatory Authority governing the service applied for or for the following <br /> reasons: <br /> <br /> (a) solely at the discretion of Company, if the Applicant's installation or <br /> equipment is known to be hazardous, potentially hazardous, or of such character <br /> that satisfactory or accurately metered service cannot be given; <br /> <br /> (b) if the Applicant is indebted for the same kind of service previously <br /> provided, including but not limited to unauthorized usage; however, in the event <br /> the indebtedness of the Applicant for service is in dispute, the Applicant will be <br /> served upon complying with the applicable deposit requirement; <br /> <br /> (c) for refusal to make a deposit if Applicant is required to make a deposit <br /> under these Service Regulations; <br /> <br /> (d) for refusal to provide verifiable identification; or <br /> <br /> (e) for changing Customer identity to avoid or evade payment of a utility bill. <br /> Company may require an Applicant to establish that the Applicant is not the <br /> same person who owes the debt to Company for service at the address or <br /> another address and that said person no longer is an occupant of the premises if <br /> one of the following conditions exists: <br /> <br /> (i) service has been disconnected st the address within the <br /> preceding thirty (30) days, or a longer period of time after March 31 and <br /> before October 31 of each year, and there is a debt owing to Company <br /> for non-payment of service at said address, <br /> <br /> (ii) a past due bill is owing at said address although service has not <br /> yet been terminated, or <br /> <br /> (iii) service has been terminated at said address because the utility's <br /> pipe, Meter, or equipment has been bypassed or tampered with, or has <br /> been determined, at the sole discretion of Company, to be inadequate to <br /> provide the total service potential of Customer. <br /> <br /> Company may withhold service to the Applicant at said address until the required <br /> information is furnished to Company. If an application for residential or commercial <br /> service is made on or after October 31 and before April 1 and more than thirty (30) days <br /> after disconnection of service, Company will initiate service unless it obtains substantial <br /> <br /> -54- <br /> <br /> <br />
