PERFORMING AGENCY shall submit claims for reimbursement on a State of Texas Pumhase Voucher (TDH Form B-13) or any other form designated by the RECEIVING AGENCY. PERFORMING AGENCY shall submit vouchers for reimbursement no later than 90 days after the end of the Attachment term. RECEIVING AGENCY shall reimburse PERFORMING AGENCY a lump sum amount upon receipt of the purchase voucher and supporting documentation, and the satisfactory inspection by RECEIVING AGENCY of the completed installation. All vouchers submitted shall include the total expenditure incurred for all services performed and equipment purchased. An expense is incurred when the service performed or material purchased is received by the PERFORMING AGENCY. Claims submitted and postmarked more than ninety (90) days following the end of the applicable Attachment term may not be reimbursed, at the discretion of the RECEIVING AGENCY. General Provisions, Terms and Conditions of Payment Article, is amended to add the following: To be eligible for reimbursement of approved equipment items, PERFORMING AGENCY shall order the equipment and incur the costs of these items before the end of the Attachment term. The invoiced equipment item(s) shall be received and paid for by PERFORMING AGENCY no later than ninety (90) days following the end of the Attachment term. If requested by RECEIVING AGENCY, PERFORMING AGENCY shall forward proof of payment and a copy of the shipping invoice showing the delivery date of any item(s) that were claimed for reimbursement by this method. General Provisions, Advance Payments Article, is not applicable to this Attachment. General Provisions, Program Income Article, is not applicable to this Attachment. General Provisions, Equipment and Supplies Article, fourth paragraph, is replaced with the following: PERFORMING AGENCY shall maintain a property inventory and submit a Report of Tangible Nonexpendable Property (TDH Form GC-11) to RECEIVING AGENCY no later than forty-five (45) days following installation. PERFORMING AGENCY shall administer a program of maintenance, repair, and protection of assels under this Attachment so as to assure their full availability and usefulness. In the event PERFORMING AGENCY is indemnified, reimbursed, or otherwise compensated for any loss of, destruction of, or damage to the assets provided under this Attachment, it shall use the proceeds to repair or replace said assets.
