DRAFT <br /> <br />F:~Attorney~ALICE\ORDWORK\CURRENT~Amend Birth & Death Fees Ord 17-5(f).wpd <br /> October 10, 2003 <br /> <br />ORDINANCE NO. <br /> <br />AN ORDINANCE OF THE CITY COUNCIL OF THE CITY OF PARIS, <br />PARIS, TEXAS, PROVIDING THAT THE CODE OF ORDINANCES, CITY <br />OF PARIS, BE AMENDED BY REVISING SUBPARAGRAPH (f) OF <br />SECTION 17-5 OF SAID CODE; PROVIDING FORAN INCREASE OF $1.00 <br />FOR THE PRESERVATION OF VITAL STATISTICS RECORDS UPON <br />ISSUANCE OF THE RECORD, INCLUDING BIRTH, DEATH, AND FETAL <br />DEATH RECORDS, IN ACCORDANCE WITH SENATE BILL 1744 WHICH <br />AMENDED THE HEALTH AND SAFETY CODE, SECTION 191.0045; <br />REPEALING ALL ORDINANCES OR PARTS OF ORDINANCES IN <br />CONFLICT HEREWITH; AND DECLARING AN EFFECTIVE DATE. <br /> <br /> BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF PARIS, PARIS, <br />TEXAS: <br /> <br /> Section 1. That the findings set out in the preamble to this ordinance are hereby in all things <br />approved. <br /> <br /> Section 2. That subparagraph (f) of Section 17-5 of the Code of Ordinances of the City of <br />Paris, Paris, Texas, be, and the same is hereby, amended so that such shall read as follows: <br /> <br />"Sec. 17-5. Registrar of vital statistics. <br /> <br /> "(f) The registrar of vital statistics is authorized and directed to furnish, upon <br />request, to any qualified applicant, a certified copy of a death certificate, recorded as <br />provided in this section, for which a fee often dollars ($10.00) will be charged for the <br />first or only copy requested, and four dollars ($4.00) for each additional certified copy <br />of the same record requested in the same request. The fee for a certified copy of a <br />birth record shall be ten dollars ($10.00), plus a two dollar ($2.00) surcharge, for a <br />total of twelve dollars ($12.00). The fee for a wallet-sized certification of birth shall <br />be ten dollars ($10.00), plus a two dollar ($2.00) surcharge, for a total of twelve <br />dollars (12.00). Such certified copy shall be issued in only such form as approved by <br />the state department of health, and receipts shall be made by the registrar and issued <br />to the applicant upon receipt of forms furnished and provided by the City. When a <br />search of birth or death records is requested, a search fee often dollars ($10.00) will <br />be charged for death certificates and a search fee often dollars ($10.00), plus a two <br />dollar ($2.00) surcharge, for a total of twelve dollars ($12.00), will be charged for <br />birth certificates. If the birth or death record is found and a copy purchased, then the <br />search fee shall not be assessed. <br /> <br /> Section 2. That all ordinances or parts of ordinances in conflict herewith are hereby expressly <br />repealed. <br /> <br />Section 3. That this ordinance shall become effective on November 1, 2003. <br /> <br /> <br />
