Paris —Lamar County Health District 400 West Sherman Street, Paris, Texas 75460 -5646 Health District: (903) 785 -4561 Women, Infant and Children (WIC): (903) 784 -1411 Fax: (903) 737 -0978 www.parislamarhealth.com Minutes of SPECIAL Meeting Paris - Lamar County Board of Health The Paris - Lamar County Board of Health held a SPECIAL meeting at 5:30 p.m. on Monday, December 151h, 2014. In attendance were Dr. Mark Gibbons, Mr. Bill Collins, Dr. Keith House, Dr. William George, Dr. Rick Erickson and Mrs. Kristi Graham. Others in attendance were Mrs. Gina Prestridge, Ms. Cheryl Johnson, Dr. Amanda Green and Mrs. Linda Kapp. Dr. Lance Wyche was not in attendance. Dr. Gibbons called the meeting to order in open session at 5:31 p.m. Mr. Collins made the motion and was seconded by Dr. House to approve the minutes from prior meeting as presented. All approved. Mrs. Linda Kapp, Certified Public Accountant with McClanahan & Holmes, advised the board members of the April — September 2014 Independent Auditor's Report and Financial Statement /Audit. Mrs. Kapp advised that there were no finding and we received a clean audit. (See attachment # 1- Independent Auditor's Report and Financial Statements). Mrs. Prestridge advised that the Paris -Lamar County Health District (PLCHD) Nurse Practitioner put in her resignation on November 19, 2014 and went to work for a private doctor. Mrs. Prestridge has contacted University of Texas at Arlington, University of Science Center of Tyler, Donna Callicoat (medical issues), LaDonna Koehn, Amy Jolly (one year to being vested at current job), Arlene Roberts, and sent an email to every Nurse Practitioner in the Paris area. Dr. Amanda Green and Dr. Gordon Strom have been filling in at the PLCHD clinic a couple of days a week. Then, Elda Francis contacted Mrs. Prestridge and advised that she had made a mistake and wants to come back to the PLCHD. Dr. Green and Mrs. Prestridge had a meeting with Mrs. Francis and she will start back at the PLCHD on December 17th, 2014. Mrs. Francis will have to wait the 90 day medical insurance probation period before her medical insurance will go into effect. Mrs. Prestridge advised that she received notification that the PLCHD would be receiving the 2 "d DSRIP payment on January 30th, 2015 in the amount of $198,938.00. Mrs. Prestridge would like to take $98,938.00 and pay off the building lease and then not have a lease note for the next 60 -100 years. The next meeting will be January 20th, 2015 at 5:30 p.m. at the Paris - Lamar County Health District. Dr. Gibbons made the motion and was seconded by Mr. Collins for the meeting to be adjourned at 6:26 p.m.
