Paris —Lamar County Health District <br />l 400 West Sherman Street, Paris, Texas 75460 -5646 <br />Health District: (903) 785 -4561 <br />pAA1S-LAR COUNTY Women, Infant and Children (WIC): (903) 784 -1411 <br />HEALTH DISTRICT <br />Fax: (903) 737 -0978 <br />www.parislamarhealtli.com <br />Minutes of Regular Meeting <br />Paris - Lamar County Board of Health <br />The Paris - Lamar County Board of Health held a regular meeting at 5:30 p.m. on Monday, November 16th, 2015. <br />In attendance were Mr. Bill Collins, Dr. Rick Erickson, Dr. Keith House, Mrs. Sharmila Franklin and Mrs. Nancy <br />Stallings. Others in attendance were Mrs. Gina Prestridge, Ms. Cheryl Johnson, Mrs. Carla Rhodes, and Mrs. Nita <br />Johnson. Dr. Mark Gibbons was not in attendance and Dr. Wyche's replacement has not been selected, <br />Mr. Collins called the meeting to order in open session at 5:30 p.m. <br />Dr. Erickson made the motion and was seconded by Dr. House to approve the minutes from the prior meeting with <br />one correction. All approved. <br />Dr. House made the motion and was seconded to elect Dr. Robert White, DDS, to complete the remaining term of <br />Dr. Lance Wyche. All approved. <br />Mrs. Prestridge advised the board on the following: <br />• We may need to consider changing the C.O.L.A. and longevity policy to be based on the annual <br />performance evaluations and if not satisfactory then only given a percentage or nothing at all. Also, is an <br />employee entitled to the full $425.00 monthly insurance premium that the Health District pays if the <br />employee works 30 hours instead of 40 hours per week? Should the $425.00 be prorated? Will discuss at <br />a later date. <br />• We will continue to meet once a month if we have a quorum. <br />• We are going to start rating all restaurants starting in January 2016. EParis Extra will host the webpage <br />and will update and post the ratings for all local restaurants. The score will have to be posted on the front <br />door of the restaurant in plain sight. Every restaurant will be given a score of A., B, C, D, or F. If not an A <br />then will be given 4 -weeks to correct the findings All D or F scores, will be given 30 days to correct all <br />findings and will have to be re- inspected. The Health District will charge a $200.00 fine if the grade is not <br />posted or visible and must be paid within 30 days. If payment is not received, then the permit will be pulled <br />and the establishment will be closed and will have to be re- inspected and pay the permit fee in additional <br />to the non - posting grade fee. Mrs. Stallings made the motion to accept the restaurant grading system and <br />Mrs. Franklin seconded the motion. All approved. Dr. Erickson made the motion that we will charged $200. <br />If the grade is not visibly posted and Dr. House seconded this motion. All approved. <br />Dr. House made the motion and was seconded by Dr. Erickson that if payment is not received, then the <br />permit will be pulled and the establishment will be closed and will have to be re- inspected and pay the <br />permit fee in additional to the non - posting grade fee. All approved. <br />
