1. Convene into QPen Session
Dr. Marc White called the meeting to order in open session at 5:40 p.m.
2. Approval of Minutes from (PRIOR meeting(s).
Dr. Jay Carpenter made the motion to approve minutes from the November 18t", 2019 meeting
and was seconded by Mrs. Susan Bellene. Motion carried. 4 ayes, 0 nays.
3. Discuss and possible act on
• Annual Evaluations on Medical Director & Executive Director
Dr. Jay Carpenter made the motion to approve the annual evaluations for Dr. Amanda
Green and Executive Director Gina Prestridge and was seconded by Mrs. Susan Bellene.
Motion carried. 4 ayes, 0 nays.
• Annual Approval of Policies and Procedures Manual and PLCHD Personnel Manual
Mrs. Susan Bellene made the motion to approve the Policies and Procedures Manual and
the PLCHD Personnel Manual and was seconded by Dr. Jay Carpenter. Motion carried. 4
ayes, 10 nays.

o Reported 0 rabies cases.
o Total liability and equity is approximately $1,642,000.

PLCHD board approved Janus 212t 2020 minutes to be submitted to the C4 Clerk in the City Hall Annex, located at 150 S.E.1st Street,
Paris, Texas, for filing.
