PARIS, That the proposal of Orkin Exterminating Company,
Inc., for the pest control service for the Paris Public
Library in the amount of $590 is hereby accepted conditioned
upon said pest control service meeting all of the terms and
conditions included in the Agreement attached hereto as
Exhibit A; and,
BE IT FURTHER RESOLVED, That the City Manager of the
City of Paris, Michael E. Malone, be, and he is hereby
authorized and directed to execute on behalf of the City of
Paris the agreement with Orkin Exterminating Company, Inc.
for pest control service for the Paris Public Library in the
form shown in Exhibit A attached hereto.

4, PAYMENT
In consideration for the service to be p~idad,bv ORKIN, the c~~~rees to pay to Orkin Exterminating C.o.mplW. Inc., its
successors or assigns, the sum of $ /stJ. ~ . plus tax of $ ~ for the initial month's service and $ n2- ,

5. OFFER EXPIRATI N: ORKIN's offer to perform these services in accordance with the terms, conditions and price stated in this
Agreement shall expire if not accepted within thirty days.
6, ORKIN'S MONEY-BACK GUARANTEE
If, during the regularly scheduled Pest Control treatments, the Customer is not completely satisfied with the results, ORKIN will return to
treat the area again at no extra charge. Should the problem persist, ORKIN will continue to work to solve the problem to the Customer's
satisfaction or ORKIN will refund the Customer's last monthly payment.
7, CUSTOMER COOPERATION
The Customer agrees to cooperate fully with ORKIN to insure effective results from the Pest Control service. In order to qualify for the
Money-Back Guarantee, the Customer agrees to maintain its premises as recommended by ORKIN in the Housekeeping and Mainte-
nance Survey Report originally provided to Customer and as recommended by ORKIN in its Pest Control Service Reports. The Customer
agrees that ORKIN may withhold any and all refunds until its recommendations have been accomplished.
8, MATERIALS
The materials used to control pests in and around Customer's premises shall conform to Federal, State and local laws and regulations,
and shall be used with all due precautions to avoid the possibility of accident to humans, domestic animals and pets. All Pest Control
work shall be performed in a safe manner and in accordance with the most modern, effective and scientific Pest Control procedures.
9, PEST DAMAGE
The Customer agrees that ORKIN is not responsible for insect or rodent damage or disruption to its operation, premises or its contents,
and the Customer specifically releases ORKIN from liability for any such claims.
10, INSURANCE
Upon request, ORKIN shall furnish to the Customer a certificate of liability insurance coverage in effect.
11. EQUIPMENT REPLACEMENT
The Customer agrees to pay to ORKIN the repair or replacement cost of any and all ORKIN equipment damaged, lost or destroyed on the
Customer's premises. Costs will be those in effect at the time of the repair or replacement.
12, TERM OF CONTRACT
This Contract shall be effective for an original period of one year. Thereafter, this Contract shall renew itself from month to month until
terminated by either party upon sixty days' written notice.
