1. Convene into Open Session
• Dr. White called the meeting to order in open session at 5:31 p.m.
2. Approval of Minutes from prior meeting(s).
• Mitzie Pirtle made a motion and was seconded by Dr. Williams to
approve the 06-01-2021 minutes. Motion carried. 5 yays, 0 nays.
3. Discuss and possible act on:
• Susan Bellene made the motion and was seconded by Dr. Putnam to
accept the applications of Dr. Myers Hurt to replace Dr. Jay Carpenter
and Angela Bedford to replace Nancy Stallings. 5 yays, 0 nays.
• Johnna McNeal presented the Annual Financial Report for the year
ending September 30,2020. Federal Expenditures are over $750,000.
so a single audit was required. (See attached report)
• No monthly reports were available.
• New business to be placed on next agenda for discussion - none.
4. Time and place of nest meeting will be Monday, October 18th, 2021 @ 5:30
p.m.
5. Adjourn meeting - Dr. Putnam made the motion and was seconded by Susan
Bellene for the meeting to be adjourned at 5:56 p.m. Motion carried. 5 yays, 0
nays.

for the PLCHD board approved September 9t", 2021 minutes to be submitted to the City Clerk in the City Hall Annex,
located at 150 S.E. 1St Street, Paris, Texas, for filing. Motion carried 5 ayes and 0 nays.
