1. Convene into Open Session
• Dr. White called the meeting to order in open session at 5:01 p.m.
2. Approval of Minutes from prior meeting(s).
• Mitzie Pirtle made a motion and was seconded by Dr. Hurt to approve
the 09-09-2021 minutes. Motion carried. 5 yays, 0 nays.
3. Discuss and possible act on:
• Public Information Request -
1) request for all active restaurants/cafes in the city of Paris,
2) Nortex Services at 546 CR 35990, Powderly, Texas, and
3) food safety routine, follow up, complaints, COVID-19 compliance
inspections for 3905 Lamar Avenue, Paris, Texas, 3420 Lamar Avenue,
Paris, Texas, 1105 NE Loop 286, Paris, Texas, 1310 Clarksville, Paris,
Texas, 2020 Lamar Avenue, Paris, Texas, 3825 Lamar Avenue, Paris,
Texas, 5280 SE Loop 286, Paris, Texas, and 630 Bonham Street,
Paris, Texas.
• DSRIP Updates - remodel with grant money, 1-2 years left to apply for
money, and able to rent more office space in the back area.
• Finished Remodel - was able to remodel the entire building on the
inside and outside and installed automatic sliding front doors
• Dr. Moore - Dr. Allison Ward -Moore is renting space and will be seeing
GYN patients.
• Monthly reports - November

4. Time and place of next meeting will be Tuesday, January 18th, 2021 (due to
MLK Holiday) @ 5:30 p.m.
5. Adjourn meeting - Susan Bellene made the motion and was seconded by
Mitzie Pirtle for the meeting to be adjourned at 5:56 p.m. Motion carried. 5
yays, 0 nays.

At the February 22, 2022, Board of Health Meeting, Dr. Williams made the motion and was seconded by Dr. Putnam for
the PLCHD board approved December 13th. 2021 minutes to be submitted to the City Clerk in the City Hall Annex, located
at 150 S.E. 1St Street, Paris, Texas, for filing. Motion carried 6 ayes and 0 nays.
