Safety Committee Meeting

Present: Ernie Clark, Melba Harris, Todd Varner, Gene Anderson, Doug Harris,
Amy Samis
Start time: 11 :00 am
Meetings will be held in the Council Meeting Conference Room
Service time on committee: 3 members for 3 yrs., 3 members for 2 yrs., 3 members for 3 yrs.
New members will serve full 3 yrs. each
· Mr. Clark will send letters to employees listed below informing them of their
recommendation for appointment on committee as well as time to be served.

City of Paris Safety Committee Meeting
May 23, 2007, 4:00 pm
City Council Conference Room
Present: Darrel Dickerson, Tim Moore, Gene Anderson, Steve Wood, Amy Samis, Jason Barber,
Melba Harris, Kelly Newman, Todd Varner, Priscilla McAnally
Chairman was elected - Dale Mayberry
Secretary was elected - Amy Samis
Safety committee was officially named "City of Paris Safety Committee"
Safety Committee will meet on the first Tuesday of each month at 3 :OOpm in council conference
room when available. When not available will meet in old council conference room upstairs.
Dale will confirm conference room availability for each meeting and reserve with Jackie lItis (Ext.
1202).
Todd will notify Michael Gott regarding adding Tim Moore to City email.
Steve Wood will be contacted through Amy Samis, via city email or telephone (Ext. 1512)
Discussed Mission Statement of Safety Committee. Todd Varner will revise Mission Statement.
Amy Samis will contact each city department to inquire about their departmental safety policy. Amy
will compile all information obtained and forward copies to each committee member by next
meeting.
Gene will contact other cities to see if they have any policies in place, if so, will get information to
Amy to add to committee members' copies of information.
Jason Barber advised The City ofMt. Pleasant safety policy was implemented by Utility Department.
Melba advised that committee should invite people in that they think had information they could
share or be assistance. Committee may want to visit the "Responsibilities" ofthe Safety Committee.
Each person has an area of expertise.
Amy will contact Matt Birch at PD and send him minutes of meeting as well.
Next Safety Committee Meeting will be Tuesday, June 5, 2007. Amy will send appointment card
to post to everyone's calendar for meeting.
Todd Varner advised committee needs to sell program from the bottom up. The more money saved,
the City can do more for employees.

Gene Anderson added, City could use money spent for claims toward things such as the consolidated
pay plan. City could reduce expenses in areas to show council. Employees look at departments and
make notes of anything unsafe and report to Todd Varner. Tell employees to report things.
Tim Moore advised that some employees in Public Works, Water/Sewer Department, abuse
W orkmans Compensation.
Amy Samis advised of employees at Public Works, primarily in the Water/Sewer Department
working very long hours (24, 48 hours straight) and not taking time off to go home to rest and sleep
due to not wanting to loose their overtime.
Melba said that it could be something that could be addressed in the Personnel Policy.
Discussion of incentives for employees for safety practices such as "Safety Bingo" (Implemented
by Paris Industries).

Present: Dale Maberry, Tim Moore, Steve Wood, Jason barber, Darrel Dickerson, Priscilla
McAnally, Matt Birch, Todd Varner.
Absent: Amy Samis, Kellie Newman, Melba Harris.
Chairman Dale Maberry called the meeting to order at 3 :02 p.m.
Chairman Maberry acknowledged all members that were present and those that were unable to
attend.
Chairman Maberry read the minutes from the previous meeting. Priscilla McAnally made a motion
to approve the minutes as presented with one correction regarding the spelling of Dale Maberry's
name. Darrel Dickerson provided the 2nd. The motion passed 6 - 0
Chairman Maberry begin discussion as to what the committee desired regarding what would
constitute a quorum. Darrel Dickerson made a motion that a quorum for this committee should be
a majority which means that there must be five member present to have an official meeting. The
motion also included that the Chairman would vote in case of a tie. Priscilla McAnally provided the
2nd. The motion passes 6 - O.
Chairman Maberry began discussion about the adoption of the Paris Safety Committee Management
Statement. Jason Barber had spent some time reviewing safety plans from other cities and presented
a Management Statement for the committees consideration. Priscilla McAnally made a motion to
accept the management statement as presented. Matt Birch provided the 2nd.
The motion passed 6 - O. NOTE: After the management statement is complete, it will be
presented to the City Manager for final approval and check to see if the statement needs to be
reviewed by the City Attorney.
Chairman Maberry began discussion about the adoption of the Paris Safety Committee mission
statement and responsibilities. Jason Barber had spent some time reviewing safety plans from other
cities and presented information with a mission statement, responsibilities and meetings. After some
discussion and slight revisions, Jason Barber made a motion to accept the information. Priscilla
McAnally provided the 2nd. The motion passed 6 - 0 NOTE: After the information is
completed, it will be presented to the City Manager for final approval and check to see if the
statement needs to be reviewed by the City Attorney.

Chairman Maberry began discussion about the adoption of City Management, Supervisors, and
Employees Safety Responsibilities. Todd Varner made one suggestion. After discussion, Jason
Barber made a motion to accept the responsibilities with revisions made. Darrel Dickerson provided
the 2nd. The motion passed 6 - O. NOTE: After the employee responsibilities information is
complete, it will be presented to the City Manager for final approval and check to see if the statement
needs to be reviewed by the City Attorney.
Chairman Maberry began discussion regarding personal safety equipment requirements and equipped
designated smoking areas. After discussion, the committee agreed that should be a departmental
decision.
Chairman Maberry began discussion about employee incentives for safety records. Suggestion of
making employee acknowledgments regarding safety, attendance, etc at a City A wards Banquet. The
committee also asked to get some input from the administration regarding length time of injury free
work and other criteria.
A goal for Amy and Todd is to finish compiling the departments that currently have safety plans.
And for those that do not, have a basic plan in place for the committee to review by the next meeting
on Tuesday July 3rd, at 3:00 p.m.

· Called meeting to order @ 3: 10 pm
· Reviewed previous meeting's minutes.
Priscilla McAnally motioned to approve minutes; Jason Barber 2nd
Minutes were approved 6 - 0
Have not received Municipal Court or Pool/Ballfield Safety Policies.
Department Heads have approved each of their department's safety plans.
Mission and Management Statements have been approved by City Manager as well as the City
Attorney.
Pull General Safety Plan and put in a book of it's own; Separate each department's plan (Each
department will get their own plan.) Will send to Carruth for approval.
· Jason Barber made motion to adopt Master Safety Plan

Master Plan Approved 6 - 0 Vote
· Discussed ideas of selling safety plan to employees; Committee members will meet with
each department to get every department represented.
· Injury Report Form will be streamline so that every department will possess the same
form.
· Discussed incentives for employees. Discussed quarterly Certificates for employees.
Will present to Carruth.

· Introduced City Manger to all committee members.
· Todd Varner has received every departments' safety policies.
· Discussed going to each department to speak with employees about their safety plans and
answer any questions. Discussed who shall be present within the departments. Each
Department will email Todd with the date of their next scheduled safety meeting.
· Discussed protocol for employees and Safety Committee members to forward safety
concerns. Dale Maberry suggested a suggestion box. Employees could write a detailed
description of safety hazard or concern and drop it in the box.
Todd Varner advised anyone could contact him on his cell phone anytime.
· Discussed confidentiality being a critical element in our safety committee meetings.
· Safety incentives for employees were discussed. Recognizing departments quarterly for
no injuries in a quarter. "Most improved" in a quarter could also be considered in
awarding a department.
· Discussion of incentives for employees. City Manager, Kevin Carruth advised that there
is 10,000 budgeted for "Employee Recognition". Mr. Carruth approved the annual
awards Banquet for City employees. This fiscal years' banquet will be in the month of
February with the theme, "We love our employees". Amy Samis will coordinate the
event.

Reviewed 15t Report of Injuries/Accidents for October
Request or ideas of any safety training in any department will be put on every agenda. Todd is
currently working on getting Sharps training for the Public Works Department.
Management Statement of Safety Policy, Safety Committee Formation, Safety Responsibilities,
were given to all committee members for their review. City Manager, Kevin Carruth has
approved and signed off on documents.
Members reviewed letter from Clarence Graves with TML dated October 9,2007. Clarence will
return on December 11,2007 to see how much progress we've made.
Request for charts, graphs, loss history, etc. Todd put together report for 4 years. The City had
$501,000 in workers compensation claims. Todd reported the information to all Department
Heads.
Melba has worked on the return to work program. She is waiting on Kevin's approval.
Todd is working on getting a contract between the City and DPS to allow the city to run
employees' drivers license.
Discussed employee drug testing. Melba advised there was a court case that an employee won
for being drug tested that it violated his fourth amendment right and he won the case in 10th
district court.
Todd, Kevin, Melba and Department Heads or Supervisors/Superintendents will meet with
employees of each department at the time of their safety meetings to distribute safety plans.
