We have been discussing the possibility of hiring a Social Worker at the Paris-Lamar County Health Department, and the position be funded by the local foundations. In this proposal is a background of the Health Department, services provided by the Health Department, staffing at the Health Department, job description for the Social Worker and a budget for the Social Worker position. After the war, the Federal Government donated the building and land to the City of Paris and County of Lamar. The City and County, along with the State of Texas, picked up the funding for the Paris-Lamar County Health Department at that time and it remains the same today. Services: The Paris-Lamar County Health Department provides many services to the citizens of Lamar County. The purpose of these services is to prevent the spread of communicable diseases in Lamar County. A list of services is provided below: 1. immunizations 2. STD diagnosis and treatment 3. Primary Health Care program for 150% poverty and below 4. Prenatal services for low income 5. Family Planning services 6. testing of public drinking water 7. chronic and acute care for low income clients 8. health education 9. vector control (mosquito spraying program) 10. rabies investigation and testing 11. restaurant inspections 12. septic system permitting and inspections 13. environmental complaint investigations 14. GENERAL DESCRIPTION: Brief Job Description: Under the supervision of the Paris-Lamar County Health Department Administrator, functions as a member of the Paris-Lamar County Health Department Team. Provide technical assistance to community leaders on strengthening local public health systems and building local capacity for public health. Consult with the community to conduct public health system assessment, assessment of health disparities, development of health improvement plans, program development, implementation and evaluation. Performs duties that support local issues such as adopting Healthy People 2010 objectives, increasing local capacity to carry out public health essential functions and coordinates and collaborates activities with staff and other local programs, and other partners in the community. Support the agency's goals, priorities and strategic plan. IV. ESSENTIAL JOB FUNCTIONS: List in order of importance the essential job functions. 30% Works with community stakeholders to facilitate development of local community partnerships. Mobilize community partnerships to identify and solve health problems. Provides leadership to assist community agencies with needs assessments, strategic planning, and capacity building. Schedules and presides at regular meetings of agencies providing health- related services and maintains meeting minutes. Participate in data collection and analysis. Provide data in readable form to advisory boards of health, coalitions, and other professionals with recommendations for action strategies. Maintain professional competency in the areas of community development, technologies, diseases, and regulations through training, workshops, and speakers to address specific workforce/community needs. 15% Secure a grant writer with expertise in the health field; facilitates the grant-writing process as needed. Research grant opportunities and local in-kind donations for community activities. Identify relevant and appropriate data information sources. 10% Provide health-related outreach and education services to the community. Informs, educates and empowers people about health issues. Collect, summarize and interpret information relevant to an issue. Collaborate with community partners to promote the health of the population. Inform communities about legislative changes and impacts. 10% Assist families in accessing health and human services. Link people to needed personal health services and assure the provision of health care when otherwise unavailable. Identify health needs of populations and provide public with access to information regarding community resources. Facilitate community solutions to address identified issues related to health services. 5% Coordinate and/or participate in health fairs for local community residents. 4% Prepare monthly reports related to established objectives and outcomes (i.e. number of contacts made, number of clients enrolled in public assistance, etc. Evaluate effectiveness, accessibility and quality of personal and population based health services. 1% Other duties as assigned. V. LICENSURE, CERTIFICATION, OR REGISTRATION PREFERRED: List the licensure, certification or registration required to perform this job: State of Texas Certified Social Worker, or State of Texas Registered Nurse or Certification in public health. Experience in public health or public administration preferred. VL KNOWLEDGE, SKILLS, AND ABILITIES: Considerable knowledge of program planning and implementations, and of policies and regulations governing the agencies' programs. Ability to communicate clearly and effectively with community agency leaders and representatives of local health foundations. Skills include communication in oral and written forms of a professional caliber, public speaking, use of audio/visual equipment including multi-media computer equipment and oraanizational skills. A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF PARIS, TEXAS; APPROVING AND SUPPORTING THE POSITION OF COORDINATOR OF COMMUNITY HEALTH SERVICES AT THE PARIS/LAMAR COUNTY DEPARTMENT OF HEALTH; MAHING OTHER FINDINGS AND PROVISIONS RELATED TO THE SUBJECT; AND DECLARING AN EFFECTIVE DATE. WHEREAS, the Paris-Lamar County Health Department has determined that there is a need in the community for a coordinator of community health services; and; WHEREAS, the St. Josephs' Community Foundation and the Paris Regional Healthcare Endowment Fund are willing to provide funds to pay for such a position; and, WHEREAS, the duties of the position of Coordinator of Community Health Services will include, but not be limited to, 1) technical assistance to community leaders on strengthening local public health systems; 2) assessment of public health disparities; 3) development of public health improvement; and 4) grant writing; and, WHEREAS, it is understood that when and/ar if, the funding from the foundation(s) is no longer available, the position would be subject to termination. NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF PARIS, TEXAS: Section 1. That the findings set out in the preamble to this resolution are hereby in all things approved. Section 2. That the City Council of the City of Paris fully approves and supports the creation of the position of "Coordinator of Community Health Services" at the Paris/Lamar County Department of Health.
