RESOLUTION NO. 2003-063
A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF PARIS,
PARIS, TEXAS, APPROVING AND AUTHORIZING THE EXECUTION OF
CONTRACT CHANGE NOTICE NO. 02 (ATTACHMENT NO. OlA) TO
THE CONTRACT FOR PUBLIC HEALTH SERVICES, TDH DOCUMENT
NO. 7560022067-2002; MAKING OTHER FINDINGS AND PROVISIONS
RELATED TO THE SUBJECT; AND PROVIDING AN EFFECTIVE DATE.
WHEREAS, the State of Texas is a funding partner with the County of Lamar and the City
of Paris for the Paris-Lamar County Health Department; and,
WHEREAS, the City Council of the City of Paris did heretofore, on the 13th day of August,
2001, in Resolution No. 2001-120, authorize the execution of a Contract for Public Health Services,
TDH Document No. 7560022067-2002, which is the conduit through which the state funds are
received; and
WHEREAS, the City Council did heretofore, on the 13th day of August, 2001, in
Resolution No. 2001-121, authorize the execution of Contract Change Notice No. 01 (Attachment
No. 04), regarding BNS - WIC Card Participation; and,
WHEREAS, said contract is in need of an additional amendment, and it is deemed
appropriate that the form of Contract Change Notice No. 02 (Attachment No. 0IA), regarding
OPHP - Local Public Health System, attached hereto as Exhibit A, be approved, and that the City
Manager be authorized to execute the same on behalf of the City of Paris, which is the authorized
contracting entity for the performing agency, the Paris-Lamar County Health Department; NOW,
THEREFORE,
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF PARIS, PARIS,
TEXAS:
Section 1. That the findings set out in the preamble to this resolution are hereby in all
things approved.
Section 2. That the form of the Contract Change Notice No. 02 to the Contract for Public
Health Services, TDH Document No. 7560022067-2002, attached hereto as Exhibit A, be, and
the same is hereby, approved.
Section 3. That the City Manager be, and he is hereby, authorized and directed to execute,
on behalf of the City of Paris, which is the authorized contracting entity for the performing
agency, the Paris-Lamar County Health Department, Contract Change Notice No. 02 (Attachment
No. 01A), under the terms and conditions and in the form shown in Exhibit A, attached hereto.
Section 4. That this resolution shall be effective from and after its date of passage.

PERFORMING AGENCY: PARIS-LAMAR COUNTY HEALTH DEPARTMENT
RECENING AGENCY PROGRAM: OFFICE OF PUBLIC HEALTH PRACTICE
TERM: September 01,2001 THRU: August 31, 2002
It is mutually agreed by and between the contracting parties to amend the conditions of
Document No. 75600220672002-01 as written below. All other conditions not hereby
amended are to remain in full force and effect.
SECTION I. SCOPE OF WORK is amended as follows:
DIRECT ASSISTANCE
State-paid positions under this contract Attachment shall perform activities as specified in
PERFORMING AGENCY'S FY 02 Service Delivery Plan.
FINANCIAL ASSISTANCE
PERFORMING AGENCY shall direct General Revenue and Preventive Health & Health
Services Block Grant funds toward the delivery of a broad range of essential services as
described in either their FY2002 General Revenue Service Delivery Plan and/or their
Preventive Health & Health Services Block Grant Service Delivery Plan.
PERFORMANCE MEASURES
For the Preventive Health and Health Services Block funds the PERFORMING
AGENCY shall complete activities as described in either their FY2002 General Revenue
Service Delivery Plan and/or their Preventive Health & Health Services Block Grant
Service Delivery Plan in the performing agency's jurisdiction no later than August 31,
2002.
SECTION II. SPECIAL PROVISIONS is amended as follows:
General Provisions, Reports Article, is amended to include the following paragraph:
PERFORMING AGENCY shall submit an Annual Expenditures Report and Activities
Report to the appropriate RECENING AGENCY Regional Director within sixty (60)
days following the end of contract period and the reports will be in a format promulgated
by the RECEIVING AGENCY.
