E. 1 St Street Plano, Texas 75460 Prepared by: Terracon Consultants, Inc. Dallas, Texas Irerracon 8901 Carpenter Freeway, Suite 100 Dallas, Texas 75247 214-630-1010 Project Overview This project consists of the remediation of hazardous materials at the Proposed Murphy Community Center including asbestos containing materials (ACM), lead based paint building components and building materials which exhibited fungal contamination. The site specific asbestos abatement design, lead abatement work plan, and fungal remediation protocol, are provided in separate sections within this document. In building work areas where asbestos abatement, lead abatement and fungal remediation overlap, the more stringent remedial work practice, based on state and federal regulations, will take precedent. It is important to note that the building has historical significance and it is the Owner's intent to preserve the identified original components of historical value throughout the remediation project. The contractor will perform the hazardous materials remediation in a sequence that ensures compliance with applicable state and federal asbestos, lead and fungal regulations. The defined sequencing will avoid combining regulated waste generated as part of the remediation. The contractor will provided a detailed schedule of the remediation sequence which will include at a minimum containment and non-containment areas, regulated areas, order in which identified hazardous materials will be remediated, waste packaging, and disposal to ensure regulatory compliance with applicable state and federal regulations. The Contractor selected to perform the remediation work will demonstrate experience performing abatement of each of the hazardous materials identified. Prior to beginning work at the site the Contractor will attend a preconstruction meeting to coordinate all site activities with the Owner and their designated representative. In addition prior to beginning any remediation activities the contractor will conduct an orientation meeting with their personnel. The purpose of the meeting will be to inform each employee of the historical significance of the structure and the specific procedures to be followed in protecting the finishes and fixtures that are in each work area. ■ ■ 1 rerracon TABLE OF CONTENTS BID FORM APPENDIX A: ASBESTOS ABATEMENT DESIGN APPENDIX B: FUNGAL REMEDITION PROTOCOL APPENDIX C: LEAD ABATEMENT 11'erracon QID FORM FOFi NAZARDQfUS MATERIALS ABATEMENT Project: Grand Theatre Company: 21 Lamar Contact: Paris, Texas Phone: Email: Terracon Project No.: 94107645 BID FORM To: Date: In compliance with your Invitation for Bids and subject to all the conditions thereof the undersigned: Name of Bidder A Corporation organized and existing under the laws of the State of , a Partnership consisting of or an Individual trading as , having principal offices in the city of , hereby proposes to furnish all labor and materials and perform all Work required for the following Project: ACKNOWLEDGMENTS: The bidder declares that he himself has examined the site of the Work and fully informed himseff regarding all pertinent conditions, and that he has examined the Scope of Work (including all Addenda received) for the work relative thereto, and that he has satisfied himself relative to the Work to be performed. 1. Based on the evidence of chronic moisture intrusion, visible water damage, and visible fungal growth Terracon documented regulated fungal contamination on the wood ceiling deck in the first floor lobby. Additionally, non- regulated fungal growth is present on water damaged ceiling tiles and ceiling tile debris in the first floor lobby. Biological-hazard contamination related to historical pigeon infestation is present as pigeon feces, and pigeon carcasses on the second floor above the lobby and within an interstitial building space located at the north end of the second floor above the lobby. The quantity and location of water damage/wood rot, fungal growth, and pigeon debris in the referenced locations is further described in Section 10.0, and the site diagrams in Appendix A. The remedial activities will be conducted in general accordance with the Texas Department of State Health Services (TDSHS) regulations contained in "The Texas Mold Assessment and Remediation Rules", (TMARR). In general, the remediation of non-regulated fungal growth will consist of the removal and disposal of drop ceiling tiles, applied ceiling tiles and ceiling tile debris in the first floor lobby. Regulated fungal growth on the water damaged wood ceiling deck will be removed by hand cleaning the wood substrate or by carbon dioxide-blasting of the wood substrate, and the ceiling deck encapsulated. The TDSHS has granted a project specific exemption related to "wood rot" associated with the wood ceiling deck and common second floor (Figure F-1, Appendix A) since the wood ceiling deck/second floor will eventually be removed, inclusive of the visible wood rot. The general debris and the pigeon debris that comprise the defined biological-hazard on the second floor will be removed and disposed as construction debris. After the general debris has been removed from the second floor visible fungal growth on the wood floor substrate will be hand cleaned or cleaned by carbon dioxide-blasting, and the floor encapsulated. The remediation work will be conducted within containment(s) under negative pressure with HEPA filtered exhaust. Terracon has documented the presence of asbestos containing materials (ACM) and lead based paint (LBP) in the walls of the first floor lobby. The abatement of ACM and LBP wiil be conducted prior to fungal remediation. The asbestos abatement specifications are provided in Scope of Work for Hazardous Materials Abatement, Terracon Project No. 94107645. 1.0 SAFETY PRACTICES The contractor is responsible for all OSHA safety practices associated with worker protection. The TMARR stipulates that a vacant building with controlled access does not require negatively pressurized, contained work areas, if only licensed individuals enter the vacant building during the duration of the fungal remediation project (Rule § 295.322, Minimum Work Practices and Procedures for Mold Remediation: (d) Containment is not required if only persons who are Ircensed or registered under this subchapter occupy the building in which the remediation takes place at any time between the start-date and stop-date for the project). The polyethylene sheeting on the ceiling of the first floor lobby will be removed to provide access to the wood ceiling deck for decontamination. A critical barrier will be installed at the second floor east demarcation wall from floor level to the ceiling deck prior to beginning any remedial work on the wood ceiling deck. b) The second floor remediation work area will be isolated from the adjacent, attached building by a double layer of six (6) millimeter polyethylene sheeting creating a containment that will be negatively pressurized. The openings between the contained areas and adjacent areas, including but not limited to doorways, corridor entrances, ventilation openings (both supply air and return air), drains, ducts and wall penetrations shall be sealed with a double layer of six (6) mil polyethylene sheeting. The containment(s) may not be removed or completely depressurized until final clearance has been achieved and written, interim notice provided by the MAC to the MRC. ii. The visual inspection criteria will consist of an evaluation of the remediation area to determine if the remediation has been properly conducted in accordance with the project specifications, applicable state regulations and the fungal growth, defined biological-hazards, and extraneous debris within the work areas has been properly removed. iii. Tape lift samples shall be collected from representative locations of the decontaminated surfaces and analyzed by optical microscopy. Any area whose surface sample test does not meet post remediation criteria will be re-tested following additional cleaning of those areas. Tape lift sample analysis will be used to assess the cleaned wood ceiling deck/second floor. iv. Tape lift analytical clearance criteria will be based on an absence of fungal structures that indicate active fungal contamination. Trace quantities of random fungal spores indicate deposition while the presence of numerous somatic and reproductive structures indicates contamination.
