AFFIDAVIT OF HEIRSHIP
Mary Ruth Folsom, being first duly swom on oath deposes imd states as foUows:

1) She is of legal ago. competent, under no legal disability and has personal knowledge of
the facts contained herein by reason of being the mother of the deceased.
2>. That 10hn Allen Guy died February 4, 2ooS at Paris Regional Hospital, Paris, Lamar
CoUnty, Texas, A resident of the county of Lamar and ståte ofTcxas. ,.
3) During his lifetime the decedent married once and only once to Doris Shanefield. the
mamage was terminated by divorce. Doris Shanefield is deceased, During the course of
- the marriage the parties had no children,
4) Othciwise during his lifetime the decedent neither adopted any children nor were any
children bo~ out of wedlock, .
