<br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> ORDINANCE NO. 95-033 <br /> <br /> AN ORDINANCE OF THE CITY OF PARIS, PARIS, TEXAS, PROVIDING THAT <br /> THE CODE OF ORDINANCES, CITY OF PARIS, BE AMENDED BY REVISING <br /> SUBPARAGRAPHS (t) AND (g) OF SECTION 17-5 OF SAID CODE; PROVIDING FOR <br /> AN ADDITIONAL $2.00 SURCHARGE FOR ISSUING CERTIr'IED COPIES OF BIRTH <br /> RECORDS AND PROVIDING FOR REMITTANCE OF A PORTION OF THAT <br /> SURCHARGE TO THE COMPTROLLER FOR DEPOSIT IN THE WORK AND FAMILY <br /> FOLICIES FIJND; REPEALING ALL ORDINANCES OR PARTS OF ORDINANCES IN <br /> CONFLICT HEREWITH; AND PROVIDING FOR AN EFFECTIVE DATE OF <br /> SEPTEMBER 1, 1995. <br /> <br /> BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF PARIS: <br /> <br /> Section 1. That subparagraphs ( fl and (g) of Section 17-5 of the Code of Ordinances of <br /> the City of Paris, Paris, Texas, be, and the same are hereby, amended so that such shall read <br /> as follows: <br /> <br /> "Sec. 17-5. Registrar of vital statistics. <br /> <br /> <br /> <br /> °(fl The registrar of vital statistics is authorized and directed to furnish, <br /> upon request, to any qualified applicant, a certified copy of a death certificate, <br /> recorded as provided in this section, for which a fee of nine dollars ($9.00) will <br /> be charged for the first or only copy requested, and three dollars ($3.00) for each <br /> additional certified copy of the same record requested in the same request. The <br /> fee for a certified copy of a birth record shall be nine dollars ($9.00), plus a two <br /> dollar ($2.00) surcharge, for a total of eleven dollars ($11.00). The fee for a <br /> wallet-sized certification of birth shall be five dollars ($5.00), plus a two dollar <br /> ($2.00) surcharge, for a total of seven dollars ($7.00). The fee for an uncertified <br /> copy of birth or death records shall be five dollars ($5.00). Such certified copy <br /> shall be issued in only such form as approved by the state department of health, <br /> and receipts shall be made by the registrar and issued to the applicant upon <br /> receipt of forms furnished and provided by the City. When a search of birth or <br /> death records is requested, a search fee of nine dollars ($9.00) will be charged <br /> for death certificates and nine dollars ($9.00), plus a two dollar ($2.00) <br /> surcharge, for a total of eleven dollars ($11.00) for birth certificates. If the birth <br /> or death record is found and a copy purchased, then the search fee shall not be <br /> assessed. <br /> <br /> "(g) The registrar of vital statistics, upon collection of a surcharge for a <br /> certified copy of a birth certificate, a wallet-sized certification of birth, or a <br /> search for a birth certificate, shall remit one dollar and eighty cents ($1.80) of <br /> that surcharge to the comptroller for deposit in the work and family policies fund. <br /> <br /> . <br /> <br /> Sechon 2. All ardinances or parts of ordinances in conflict herewith are hereby <br /> expressly repealed. <br /> <br /> Section 3.
