<br /> <br /> <br /> <br /> <br /> <br /> ORDINANCE N0. 92-041 <br /> <br /> <br /> AN ORDINANCE OF' THE CITY OF PARIS, PARIS, TEXAS, <br /> PROVIDING THAT THE CODE OF ORllINANCES, CITY OF PARIS, BE <br /> AMENDED BY REVISING SECTION 17-5 OF SAID CODE; PKOVIDING THAT <br /> THE CITY CLERK IS THE REGISTRAR OF VITAL STATISTICS FOR THE <br /> CITY; REPEALING ALL ORDINANCES UR PARTS OF ORDINANCES IN <br /> CONFLICT HEREWITH; AND PROVIDING FOR AN EFFECTIVE DATE OF <br /> SEPTEMBER 14, 1992. <br /> BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF PARIS: <br /> Section 1. That Section 17-5 of the Code of <br /> Ordinances of the City of Paris, Paris, Texas, be amended so <br /> that such shall read as follows: <br /> <br /> "Sec. 17-5 Registrar ot vital statistics. <br /> (a) The City Clerk shall be the local <br /> registrar of vital statistics for the City. Each <br /> local registrar shall appoint a deputy registrar so <br /> that a registrar will be available at all times for <br /> the registration of births and deaths. <br /> (b) The local registrar of vital statistics <br /> shall perform the duties of that office as provided <br /> for by city ordinance, state law and by such rules <br /> and regulations as may be promulgated by the state <br /> board of health as provided by law. <br /> <br /> (c) The clerk ur such other competent <br /> employee as the city clerk may designate, shall <br /> perform the clerical work required of the local <br /> registrar of vital statistics. The clerk or other <br /> employee performing such duties shall be a notary <br /> public for the purpose of acknowledgment of <br /> signatures, certification of documents and the <br /> administration of oaths necessary in the <br /> performance of official duties. <br /> (d) The local registrar of vital statistics <br /> shall be supplied with permanent record books, in a <br /> form approved by the state registrar, tor recording <br /> all births, deaths, and stillbirths occurring <br /> within the City. She shall make a camplete and <br /> accurate copy of each birth, death and stillbirth <br /> certificate registered by her or the deputy <br /> registrar, in a record book referred to above, <br /> which shall be preserved permanently in her office <br /> as the official recorcf of the city, in such manner <br /> as directed by the state registrar. <br /> (e) No fee shall be paid to the local <br /> registrar of vital statistics for registering and <br /> recording certificates of birth, death and <br /> stillbirth. <br /> (f) The local registrar of vital statistics <br /> is authorized and directed to furnish upon request <br /> to any qualified applicant a certified copy of a <br /> death certificate recorded as provided in thi.s <br /> section for which a fee of nine dollars ($9.00) <br /> will be charged for the first or only copy <br /> requested and three dollars ($3.00) for each <br /> additional copy of the same record requested in the <br /> same request. The fee for a certified copy of a <br /> birth record shall be nine dollars ($9.00). <br /> Additional copies sha.ll be nine dollars ($9.00) for <br /> each copy requested. Such a certified copy shall <br /> be issued in only such form as approved by the <br />
