RESOLUTION NO.
A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF PARIS,
PARIS, TEXAS, APPROVING AND AUTHORIZING THE EXECUTION OF
A MEDICAL TRANSPORT AGREEMENT WITH DUBUIS HOSPITAL OF
PARIS; MAKING OTHER FINDINGS AND PROVISIONS RELATED TO
THE SUBJECT; AND PROVIDING AN EFFECTIVE DATE.
WHEREAS, Dubuis Hospital of Paris has requested that the City of Paris, by and through
its Emergency Medical Service, provide non-emergency transport of patients to and from its facilities;
and,
WHEREAS, the form of the contract with Dubuis Hospital of Paris, attached hereto as
Exhibit A, should, in all things, be approved, and the Mayor should be authorized to execute the
same; NOW, THEREFORE,
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF PARIS, PARIS,
TEXAS:
Section 1. That the findings set out in the preamble to this resolution are hereby in all things
approved.
Section 2. That the form of the contract between the City of Paris and Dubuis Hospital of
Paris for non-emergency transport of patients to and from its facilities, attached hereto as Exhibit A,
be, and the same is hereby, approved.
Section 3. That the Mayor be, and he is hereby, authorized and directed to execute, on behalf
of the City of Paris, the contract with Dubuis Hospital of Paris, under the terms and conditions and
in the form shown in Exhibit A, attached hereto.
Section 4. That this resolution shall be effective from and after its date of passage.

MEDICAL TRANSPORT AGREEMENT

) KNOW ALL MEN BY THESE PRESENTS:
COUNTY OF LAMAR )
THAT, the CITY OF PARIS, PARIS, TEXAS, a municipal corporation, acting by and
through its Mayor, Michael J. Pfiester, duly authorized, hereinafter called CITY, and DUBUIS
HOSPITAL OF PARIS, acting by and through its Administrator, Berry Gilbert, duly authorized,
hereinafter called HOSPITAL, do hereby contract and agree as follows:
1. CITY agrees to furnish transportation of patients to or from the Dubuis Hospital of Paris at
the HOSPITAL'S request.
2. CITY will bill HOSPITAL at the customary rate for transport and supplies, with HOSPITAL
being entitled to a twenty percent (20%) discount if payment to CITY is made within fifteen
(15) days of the billing date.
3. In the event HOSPITAL has not paid bill within thirty (30) days of billing date, one hundred
percent (100%) of bill will accrue interest at the statutory limit.
4. It is hereby mutually agreed that either party has the right to unilaterally terminate this
agreement after having given thirty (30) days written notice to the other party.
