A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF PARIS, PARIS, TEXAS, APPROVING CHANGE ORDER NO. ONE TO CONTRACT FOR THE CONSTRUCTION OF THE NEW ANIMAL SHELTER; ACCEPTING A DONATION OF $40,000.00 FROM THE MARY COOKE ELLIS TRUST ON BEHALF OF THE LAMAR COUNTY HUMANE SOCIETY IN FURTHERANCE OF SAID PROJECT; MAKING OTHER FINDINGS AND PROVISIONS RELATED TO THE SUBJECT; AND DECLARING AN EFFECTIVE DATE. WHEREAS, the City Council of the City of Paris did heretofore, on the 12th day of August, 2002, in Resolution No. 2002-129, award the contract for the "City of Paris Animal Shelter Project" to Reeves Construction Services, Inc.; and, WHEREAS, the City Engineer, Shawn Napier, acting as project engineer, recommends the amendment of the contract for said project to reflect changes as described on Exhibit A attached hereto, and providing for a net decrease of $26,674.00 in the contract price for a total contract price of $414,826.00; and, WHEREAS, the form of Change Order No. One, attached hereto as Exhibit A, should, in all things, be approved, and the Mayor should be authorized to execute the same; NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF PARIS, PARIS, TEXAS: Section 1. That the findings set out in the preamble to this resolution are hereby in all things approved. Section 2. That the form of Change Order No. One to the Contract for the "City of Paris Animal Shelter Project," attached hereto as Exhibit A, for the said project to reflect changes as described on Exhibit A attached hereto, and providing for a net decrease of $26,674.00 in the contract price for a total contract price of $414,826.00, be, and the same is hereby, approved. Section 3. That the Mayor be, and he is hereby, authorized and directed to execute, on behalf of the City of Paris, said Change Order No. One, upon the terms and conditions and in the form shown in Exhibit A, attached hereto. Section 4. That a donation of $40,000.00 from the Mary Cooke Ellis Trust on behalf of the Lamar County Humane Society in furtherance of said project is hereby accepted with the thanks and appreciation of the City Council and the citizens of the city of Paris. Section 5. That this resolution shall be effective from and after its date of passage. PASSED AND APPROVED this 12th day of May, 2003. City Engineer DATE: May 8, 2003 SUBJECT: Animal Shelter Change Order No. One The following list are change order items that have been submitted by the contractor: 1. Entry Ceiling $643 2. Stud walls to CMU $507 3. Insulated glass $589 4. Test hold excavation $3,212 5. Added Floor Drain $746 6. Slab fill change $1,432 7. Added window $179 8. Concrete at trenches $955 9. Steel Deck $2,966 10. Extend walls to roof $10,742 A full description is attached for the items listed above. The total change order amount for these items is $21,971.00. There are three deductions involving the animal shelter. The first deduction is that the Humane Society will be donating $40,000 to buy kennels. The second is cabinets, shelving, a small slab and fencing items will be taken out of the items to be constructed. The net deduction for these items total $48,645.00. The overall change order will be a deduction of $26,674. cc: Michael E. Malone, City Manager Larry W. Schenk, City Attorney Karl Louis, Chief of Police EXHIBIT Animal Shelter Change Order Items 1. Entry ceiling - No provision was in the plans to install a hard deck surface over the entryway area. 2. Stud walls to CMU - A small wall in the dog mn area was listed to be sheetrock, this will change that portion to CMU blocks. 3. Insulated glass - This is a requirement from the Uniform Building Code (UBC). 4. Test hole excavation - This was done due to the fact that this was a dumpsite from the 1916 Paris fire. Two areas of debris were encountered and removed. 5. Added Floor Drain - a floor drain was added in the euthanasia room. 6. Slab fill change - a select material was used to backfill the entire slab area, the e~cavated test holes and compacted. This area is beneath the sand pad 7. Added window - this window will be in the reception area. 8. Concrete at trenches - this is to provide U-shaped rebar in the trench drains. 9. Steel deck - This is to provide a hard surface over the personnel portion of the building. 10. The additional wall height is to separate the quarantine areas, operating room and personal areas from the regular kennel areas.
