What is the specific problem to be addressed by this application? <br /> <br />2. Explain the nature and extent of the problem using verifiable statistics. <br /> <br />3, What resources are currently being used in the applicant's geographic area to address this <br />problem and how do those resources work together? <br /> <br />4, Identify the gap in available resources or services that makes this application necessary. <br /> <br />5. If funded, how would the proposed project work with the community and with other agencies <br />toward impacting the problem stated above? <br /> <br />6. What are the specific activities proposed for this project? Please include information on target <br />area, population, and number of people served. (If a continuation application, state only what activities <br />differ from the current year of funding.) <br /> <br />7. Explain how the proposed project activities will address the stated problem. <br /> <br />8. List the measures that the project will use to determine the effectiveness of the project and its <br />impact on the stated problem. <br /> <br />9.
