<br />" <br /> <br />December 1, 1996: a current job description, an organizational chart depicting the position, <br />identification of the position's direct supervisor, a current performance evaluation, and a statement <br />of health outcomes toward which the position's activities are directed. <br /> <br />These positions report to and are directly supervised by PERFORMING AGENCY administrative <br />staff. Supervision authorization includes, but is not limited to, overseeing daily work assignments <br />and duties, staff development, evaluations, daily supervision, leave approval, promotions, and <br />disciplinary actions including termination of the employee. <br /> <br />FINANCIAL ASSISTANCE <br /> <br />PERFORMING AGENCIES receive 73.5 percent of their fmancial assistance from General <br />Revenue funds and 26.5 percent from the Preventive Health and Health Services Block Grant. <br />PERFORMING AGENCY is required to submit to the appropriate Public Health Regional <br />Director a report detailing how these funds are currently being used by December 1, 1996. This <br />report must identify activities and objectives toward which these funds are directed. <br /> <br />For FY 1998, while General Revenue funds will continue to be directed toward the delivery of <br />a broad range of essential public health services, the Preventive Health and Health Services Block <br />Grant funds will be designated for activities directed solely toward the prevention of heart disease <br />and cancer. <br /> <br />During FY 1997, PERFORMING AGENCY agrees to develop two written plans for service <br />delivery in FY 1998, one plan for the General Revenue support and one plan for the Preventive <br />Health and Health Services Block Grant support. These plans must include a statement of <br />outcome objectives, specific activities to be performed, and a budget that links these objectives <br />and activities to the prescribed percentages. Tlus plan must be submitted to the appropriate Public <br />Health Regional Director by February 1, 1997. <br /> <br />ASSESSMENT, NOTICES, AND TRAINING: <br /> <br />PERFORMING AGENCY will assess immunization levels in the clinical records of preschool age <br />children. Immunization records will be randomly selected and assessed using the Center for <br />Disease Control and Prevention (CDC) Clinic Assessment Software Application (CASA) available <br />from RECEIVING AGENCY, Immunization Division. Two assessments using the criteria <br />specified in attached Exhibit A must be completed during the term of this Attachment. One of <br />these assessments may be conducted in cooperation with RECEIVING AGENCY. Assessment <br />results must be reported to RECEMNG AGENCY Program, Director, Immunization Division, <br />not more than two weeks after the assessments are completed for inclusion in a quarterly summary <br /> <br />ATTACHMENT - Page 2 <br />
