The electronic <br />file (either a backup or transfer file) for each clinic assessment should contain the following <br />information: <br /> <br />.. <br /> <br />Date of assessment <br />Name and address of assessment site (including county and TDH Region) <br />Contact name (preferably the name of individual that performed assessment) <br />Contact organization name <br />Site(s) covered by filing system <br />Description of files that were used for the assessment (e.g., card files, medical <br />records) <br />Criteria for excluding records (if any) <br />Estimated "active" client population, sampling interval and sample size <br /> <br />.. <br />.. <br />.. <br /> <br />.. <br /> <br />.. <br /> <br />.. <br /> <br />.. <br /> <br />PERFORMING AGENCY will produce reminders of upcoming immunizations as well as recall <br />for children who are due or overdue for doses of vaccine. The notifications may be automated <br />or manual and may include mailed or telephone messages. Extra effort should be made to notify <br />parents or guardians of children at high-risk of failure to complete the series on schedule (e.g. <br />children who start their series late). <br /> <br />INVESTIGATIONS/CONTROL MEASURES <br /> <br />PERFORMING AGENCY will investigate all reported cases and suspected cases of invasive <br />Haemophilus influenzae disease in children < 5 years of age and all reported cases and suspected <br />cases of measles, rubella, pertussis, paralytic poliomyelitis, and diphtheria within 24 hours of <br />receipt of the initial case report. PERFORMING AGENCY will investigate all reported <br />suspected cases of mumps and tetanus within 48 hours of receipt of the initial case report. <br /> <br />PERFORMING AGENCY will complete all case investigation forms and provide complete <br />epidemiologic data on all reported cases of invasive Haemophilus influenzae disease in children <br />< 5 years of age, and all reported cases of measles, mumps, rubella, congenital rubella <br />syndrome, pertussis, diphtheria, and paralytic poliomyelitis to the RECEIVING AGENCY, <br />Immunization Division, within 30 days of the initial case report. PERFORMING AGENCY will <br />provide copies of investigation forms to the RECEIVING AGENCY program. <br /> <br />PERFORMING AGENCY will adhere to the Vaccine-Preventable Disease Surveillance Guidelines <br />provided by the RECEMNG AGENCY program. PERFORMING AGENCY will implement <br />the most current outbreak control procedures and measures as recommended and provided by the <br />RECEIVING AGENCY program. <br /> <br />ATTACHMENT - Page 3 <br />
