N'C x:r :;, <br /> <br />4, PAYMENT <br />In consideration for the service to be p~idad,bv ORKIN, the c~~~rees to pay to Orkin Exterminating C.o.mplW. Inc., its <br />successors or assigns, the sum of $ /stJ. ~ . plus tax of $ ~ for the initial month's service and $ n2- , <br />plus tax of $ i!>U'1 per month for the following eleven (11) months. P yment shall be due upon receipt of invoice. <br />5. OFFER EXPIRATI N: ORKIN's offer to perform these services in accordance with the terms, conditions and price stated in this <br />Agreement shall expire if not accepted within thirty days. <br />6, ORKIN'S MONEY-BACK GUARANTEE <br />If, during the regularly scheduled Pest Control treatments, the Customer is not completely satisfied with the results, ORKIN will return to <br />treat the area again at no extra charge. Should the problem persist, ORKIN will continue to work to solve the problem to the Customer's <br />satisfaction or ORKIN will refund the Customer's last monthly payment. <br />7, CUSTOMER COOPERATION <br />The Customer agrees to cooperate fully with ORKIN to insure effective results from the Pest Control service. In order to qualify for the <br />Money-Back Guarantee, the Customer agrees to maintain its premises as recommended by ORKIN in the Housekeeping and Mainte- <br />nance Survey Report originally provided to Customer and as recommended by ORKIN in its Pest Control Service Reports. The Customer <br />agrees that ORKIN may withhold any and all refunds until its recommendations have been accomplished. <br />8, MATERIALS <br />The materials used to control pests in and around Customer's premises shall conform to Federal, State and local laws and regulations, <br />and shall be used with all due precautions to avoid the possibility of accident to humans, domestic animals and pets. All Pest Control <br />work shall be performed in a safe manner and in accordance with the most modern, effective and scientific Pest Control procedures. <br />9, PEST DAMAGE <br />The Customer agrees that ORKIN is not responsible for insect or rodent damage or disruption to its operation, premises or its contents, <br />and the Customer specifically releases ORKIN from liability for any such claims. <br />10, INSURANCE <br />Upon request, ORKIN shall furnish to the Customer a certificate of liability insurance coverage in effect. <br />11. EQUIPMENT REPLACEMENT <br />The Customer agrees to pay to ORKIN the repair or replacement cost of any and all ORKIN equipment damaged, lost or destroyed on the <br />Customer's premises. Costs will be those in effect at the time of the repair or replacement. <br />12, TERM OF CONTRACT <br />This Contract shall be effective for an original period of one year.
