<br />NOT APPLICABLE TO 111/5 GRANT <br /> <br />n. INDIVIDUAL PROGRAM PLAN AND FAMILy INvOL VEMENT <br /> <br />A. Eac.b cliem placed with the Service Agency shaD have a written Individualized PTOgram Plan (IPP) developed <br />in concert with the cliem and llIUtuaIIy agreed upon by the appropriate Service Agency sr.jf and appropriate <br />county personnel prior to placement <br /> <br />D. Copies of the original IPP and the periodic reviC\\~ are to be maintained by the Service Agency and the COUnty <br />placement officer. <br /> <br />B. The IPP shaD be reviewed jointly by the apPropriate Service Agency ~ the child, and the appropriate county <br />peJ'sonneJ at reasonable intervals, DOt to exceed ninety (90) days, to assess the cliem's Pl'Ogress with modifiCation <br />of the IFP being made when indicated. <br /> <br />C. The IPP shaD contain the reasons why the Placement will bCOefu the client; shaD speciry behavioral goals and <br />objectives being sought for each clien~ shaD Slate how the goals and objectives are to be achieved in the Service <br />Agency P!acemen~ and sha11 Slate how Jlarent(s), 8Uardian(s), and where POssible, grand parents other <br />extended family mem~ will be involved in the IPP to assist in preventing or controlling the client's alleged <br />delinquem behavior or alleged conduct indiCation a need for SUpervision as defined in the Texas Family Code. <br /> <br />E. The Service Agency shaD Provide each child's probation officer with a IlTitten report of the child's Progress on <br />a monthly basis. <br /> <br />F. If a child in Placement at the Service Agency makes an llIIauthorized departure, becomes seriously ill. or is involved <br />in an incident of a seriOUs nature, the Service Agency shaD n~ the child's probation officer and the COUnty placemem <br />officer immediately and ensure that parents and proper authorities, including the Texas DC)lartment of Protective and <br />Regulatory Services, are notified. <br /> <br />G. The COUnty TeSelves the right to lenninate the child's placement at the Service Agency at its description. The <br />Service Agency IJlUst not release a child to any)lerson or agency other than the COUnty without the express consent of <br />the county. <br /> <br />L Unless otherwise stipulated by the county, the child may visit freely with parents and relatives at the hODle in <br />accordance with established Service Agency POlicies." <br /> <br />H. The county IJlUst approve the child's participation in any furloughs, home visits or extended agency trips. . <br /> <br />J. SIISpected or alleged cases of child abuse IJlUst be inunediateJy repotted to the COUnty placemem officer and the <br />Texas ~ent ofProlective and Regulatory Services. <br /> <br />IlL EXAMINATION OFPROGRAM AND REcORDS <br /> <br />A. The Service Agency agrr:s that it wiI1 pertuit the COUnty to CXaIninc and evalllale its PTOgram of Services provided <br />under the lenns of this ConlIact and to review COUnty cliem records. This examination and evaluation of the program <br />will include unscheduJcd site visitations, Observation of progIams in O]leralion, interviews, and the administralion of <br />questionnaires to the. .~.o~~_S~ceAgency and the client. <br /> <br />- .
