<br />Funeral leave is paid time off. An employee may arrange additional time off with the approval of <br />his/her Department Head. Any additional time off will be charged to, and limited by any accrued <br />leave time. Substantiation of an employee's use of funeral leave may be required. <br /> <br />SECTION 14.10 - Authorized Leave Without Pay <br /> <br />Employees may be authorized to take leave without pay for up to three (3) months during a 12 month <br />period; however in unusual circumstances, the City Manager has the option of approving up to an <br />additional three (3) months ofleave without pay. Leave without pay may be taken under such terms <br />and conditions as may be mutually agreeable between the employee, his/her department head and the <br />City Manager. After exhausting all authorized leave without pay, if the employee is not able to <br />perform the essential job functions, the employee will be terminated. <br /> <br />A request for such leave must be made in writing to the employee's Department Head. Length of <br />service, quality of performance, the urgency of need and the City's interest will be considered prior <br />to approving the request. If an employee is eligible for Family and Medical Leave and has exhausted <br />all paid leave benefits, leave without pay must be approved. <br /> <br />Fringe benefits accrued at the time of departure will be carried over to the time of return. However, <br />no fringe benefits will be accumulated during the period of absence. <br /> <br />Employees must notify the department head of his /her intention to return to work from authorized <br />leave without pay as soon as possible. Not returning to work within three (3) working days after <br />expiration of a leave of absence is considered a resignation. <br /> <br />SECTION 14.11- Injury Leave <br /> <br />Civil Service employees must be granted injury leave with full pay, for up to one (1) year as required <br />by Civil Service Law or until the employee is able to return to duty, whichever comes first. Injury <br />leave applies only to Civil Service Employees. <br /> <br />SECTION 14.12 - Worker's CompensationlReturn to work <br /> <br />Employees who are injured on the job must report the injury to their supervisor immediately but no <br />later than 24 hours of injury. The supervisor must complete the City of Paris Supervisor's First <br />Report ofInjury form immediately upon receiving a report of injury from the employee and submit <br />to the Finance Office. <br /> <br />After an injured employee is off the job for five days and is unable to return to full duty, he/she will <br />receive Worker's Compensation benefits only, unless the employee elects to <br /> <br />38 <br /> <br />Revised 01.25.05 <br />
