<br />:.:.,:/.'\ <br /> <br />supplement Worker's Compensation with paid leave. Worker's Compensation may be supplemented <br />with all accrued leave with the exception of sick leave. The employee may also choose to use paid <br />leave, other than sick leave, for the first five (5) days that he/she is off the job. After all vacation, <br />comp, holiday or attendance holiday leave is exhausted, the employee will receive only Worker's <br />compensation which will not be paid at 100% of the employee's salary. The supervisor may <br />periodically require the employee to provide verification of his /her inability to perform the essential <br />job functions. <br /> <br />The City of Paris is committed to providing a safe place for our employees to work. Our early return <br />to work program provides opportunities for an employee who is injured on the job to return to work <br />at full duty. If the injured worker is not physically capable of returning to full duty, the program <br />provides opportunities to perform his or her regular job with modifications or, when available, to <br />perform alternate temporary work that meets the injured worker's physical capabilities. When the <br />manager is considering alternate temporary work, he/she should contact the HR Director or designee <br />for approval. <br /> <br />SECTION 14.13 - Health Insurance <br /> <br />Health insurance is available to all regular full-time employees. The cost of employees' insurance <br />benefits will be established by the City Council. This coverage begins (90) days from the date of <br />employment. The company providing coverage will be made known to employees at the time of <br />employment. If necessary, the City reserves the right to substitute another carrier with similar <br />insurance. Complete details on the extent of coverage are available in the Human Resources Office. <br />Coverage is available for dependents at the employee's expense. <br /> <br />SECTION 14.14 - Life Insurance <br /> <br />Life insurance is available to all regular full time employees. This coverage begins ninety (90) days <br />from the date of employment. If necessary, the City reserves the right to substitute another carrier <br />of similar insurance. Complete details of the life insurance will be made available to each employee. <br />The cost of the premiums for life insurance will be paid entirely by the City. <br /> <br />SECTION 14.15 - Continuation of Health Insurance after Termination <br /> <br />The Consolidated Omnibus Budget Reconciliation Act 1986 (COBRA) gives workers and their <br />families who lose their health benefits the right to continue group health benefits provided by their <br />group health plan for limited periods of time under certain circumstances such as voluntary or <br />involuntary job loss, death, divorce and other life events. Qualified individuals will be required to <br />pay the entire premium for coverage up to 102 percent of the cost of the plan. Each <br />employee/dependent losing coverage due to a qualifying life event will receive a COBRA notice. <br /> <br />Employees who are terminated due to gross misconduct are not eligible to receive COBRA benefits. <br /> <br />39 <br /> <br />Revised 01.25.05 <br />
