STATEMENT OF WORK: <br /> <br />Contractor shall deliver comprehensive health care services to eligible low-income individuals as <br />authorized under the Texas Health and Safety Code, Chapter 31, Texas Primary Health Care (PHC) <br />Services Act. The following priority services are the services the contractor agrees to offer: (1) <br />diagnosis and treatment; (2) emergency services; (3) family planning services; (4) preventive health <br />services, including immunizations; (5) health education; and (6) laboratory, x-ray, nuclear medicine, <br />or other appropriate diagnostic services. <br /> <br />Contractor shall comply with all applicable federal and state laws, rules, regulations, standards, and <br />guidelines in effect on the beginning date ofthis contract Attachment unless amended, including but <br />not limited to: <br /> <br />. DSHS Primary Health Care (PHC) Services Rules, 25 T AC, Chapter 39, 9939.1-39.11; <br />and <br />. Texas Primary Health Care Services Act, Health and Safety Code, Chapter 31. <br /> <br />The following documents are incorporated by reference and made a part ofthis contract Attachment. <br />These include: <br /> <br />. DSHS FY08 Primary Health Care Competitive Request for Proposals and any revisions; <br />. Contractor's FY08 Primary Health Care Application and any revisions; <br />. Policies and Procedures Manual for Primary Health Care, revised for FY08, or latest <br />reVISIOn; <br />. Department of State Health Services Standards for Public Health Clinic Services, revised. <br />August 2004, or latest revision; <br />. DSHS Policy and Procedures Manual for Primary Health Care, and any revisions; <br />. DSHS Core Tool On-Site Evaluation Report, revised for 2007, and Core Tool <br />Monitoring Instructions, FY07, or latest revision; and <br />. DSHS Primary Health Care On-Site Evaluation Report, revised for 2007, and PHC Tool <br />Monitoring Instructions, FY07, or latest revision. <br /> <br />Within thirty (30) days of receipt of an amended standard(s) or guideline(s), Contractor shall inform <br /> <br />PROGRAM A TT ACHMENT - Page 1 <br />
